Chapter 34.3, Problem 1CSC

If you have sleep apnea, you never get a good night’s sleep. Although you may not realize it, your sleep is briefly interrupted from 30 to more than 100 times each night when you temporarily stop breathing for 10 to 60 seconds (“apnea” means “without breath”). Your blood O2 falls and CO2 rises, causing you to partially awaken, often with a snort. What causes this? Obstructive sleep apnea, the most common form, occurs when muscles that support the tongue and soft tissues of the pharynx relax, causing these tissues to collapse and obstruct the airway. People who snore, are overweight, drink alcohol before bed, or smoke are all more susceptible to sleep apnea. The resulting daytime drowsiness, making it hard to concentrate and dangerous to drive, is bad enough. But the repeated O2 deprivation and CO2 buildup caused by sleep apnea puts your heart at risk as well, so this is a condition to take seriously. How does your body know that you have stopped breathing?

PROFILE OF PATIENT Mr. Asare is a 75-year-old male admitted with an exacerbation of chronic obstructive pulmonary disease (emphysema) at Korle-Bu Teaching Hospital. He has been keeping the head of the bed up for most of the day and night to facilitate his breathing which has resulted in lower back pain. Acetaminophen (Tylenol) was not effective in reducing his pain, so the health care provider has prescribed oxycodone/acetaminophen (Percocet) one to two tablets PO every four to six hours as needed for pain. Mr. Asare is on 2 liters of oxygen by nasal cannula. He can receive respiratory treatments of albuterol (Proventil, Ventolin) every six hours as needed. Mr. Asare needs someone to walk beside him when he ambulates because he has an unsteady gait and often needs to stop to catch his breath.   CASE STUDY The nurse enters the room and finds Mr. Asare hunched over his bedside table watching television. He says this position helps his breathing. His lung sounds are clear but diminished…
Which of the following statements about type I respiratory failure is correct? CNS depression with reduced respiratory drive can lead to type I respiratory failure. The partial pressure of carbon dioxide in the arterial blood is above 45 mmHg. It is caused by gas-exchange respiratory failure. Medical ventilator should be used in patients with type I respiratory failure. Hypercapnia occurs prior to hypoxemia.
Which statement concerning the clinical manifestations of asthma is CORRECT? Question 18 options: tachypnea is common and can cause respiratory failure hypoxemia may occur as distal airways narrow bradycardia is a normal homeostatic response to SOB decreased vascular permeability leads to laryngeal edema
