Case summary:
In the given case, the patient Brian Rhen was found holding his head on the side of the road at the bus crash site. A severe headache and nausea were reported by him. After several days, he reported vertigo, and therefore, was referred to a neurologist. He was diagnosed with BPPV (benign paroxysmal positional vertigo). One of the causes of this is the dislodgment of the otoliths from the otolithic membrane. In this case, Mr. Brian experienced vertigo when he rolled over in bed, while sitting up, or while turning head from side to side.
Characters in the case:
Brian Rhen.
Adequate information:
Certain positions, such as rolling, sitting up, or moving head side to side, provoke vertigo in Mr. Brian.
To determine:
Based on the triggering positions of vertigo, the part of the vestibular apparatus that may be affected by the displaced otoliths.

- Julio D., who had recently retired, was enjoying an afternoon of playing golf when suddenly he experienced a severe headache and dizziness. These symptoms were quickly followed by numbness and partial paralysis on the tipper right side of his body, accompanied by an inability to speak. After being rushed to the emergency room, Julio was diagnosed as having suffered a stroke. Given the observed neurological impairment, what areas of his brain were affected?arrow_forwardA 78-year-old male presents for a hospital follow-up, post-stroke, two days ago. During the neurologic portion of the physical exam, the patient cannot feel the cotton tip of an applicator as you brush it along his left forehead, cheek, and jaw on the left side of his face. Based on this scenario and phsyical exam findings, which of the following cranial nerves is most likely affected? - A 40 year old male has just been diagnosed with amyotrophic lateral sclerosis, better known as Lou Gehrig’s Disease. A recent MRI has shown significant demyelination of the gracile nucleus of the dorsal columnar-medial lemniscal pathway. He has also noted progressive difficulty walking, he lifts each foot high and stamps it forcefully on the ground with each step. He must watch his feet while walking to avoid striking doors or other obstacles with them. The disorder producing these symptoms most likely involves which of the following structures? - When their second child was born, Kiko and Taka were told their baby had spina bifida and would be kept in intensive care for a week. After that time, a “shunt” would be put in. Also, immediately after the birth, an operation was performed on the infant’s lower back. The parents were told that this operation went well but that their son would always be a “little weak in the ankles.” Explain the statements in quotation marks in more informative and precise language.arrow_forwardThe following event received worldwide attention in 1962: A boy playing in a train yard fell under a train, and his right arm was cut off cleanly by the wheels. Surgeons reattached the arm, sewing nerves and vessels back together. The surgery proceeded very well. The arm immediately regained its blood supply, yet the boy could not move the limb or feel anything in it for months. Explain why it took longer to reestablish innervation than circulation.arrow_forwardAn individual has a spinal cord injury superior to the area involved in the patellar reflex circuit such that all communication to the brain is broken off. How would this affect the operation of the patellar reflex circuit?arrow_forward
