Treatment Plan Diagnosis Instruments/Screens to Facilitate Diagnosis: To fully establish and get a better baseline of Nestor's porn consumption, administer the Cyber Pornography Addiction Test. Cyber Pornography Addiction Test (CYPAT) is a self-report screening scale composed of 11 items scored on a five-point Likert scale with essential psychometric properties (Weeks, 2022). The personality Assessment Inventory is another essential assessment when diagnosing and developing a therapeutic plan. The personality assessment inventory is a self-administered, objective test of personality and psychopathology designed to provide information on critical client variables in behavioral healthcare settings (Morey, 2004). This assessment gives more understanding of a person's psychopathology and offers tangible feedback for diagnoses. Becks Depression Inventory, The Beck Depression Inventory (BDI) is widely used to screen for depression and to measure behavioral manifestations and severity of depression. This assessment can be used on individuals 13-80 years of age. The BDI is a 21 self-report item, which individuals complete using multiple-choice response formats. The BDI takes approximately 10 minutes to complete. The validity and reliability of the BDI have been proven promising across many individuals and cultures. Alcohol Problems Questionaire, The Alcohol Problems Questionnaire (APQ) was designed as a clinical instrument for measuring alcohol-related problems. It has been used to study the relationship between alcohol-related problems and dependence within the bi-axial model (Williams & Drummond, 1994). DSM-5 Diagnosis: At least one of the three following symptoms must be experienced on almost all or all (approximately 75%–100%) occasions of sexual activity (in identified situational contexts or, if generalized, in all contexts): Considerable difficulty in obtaining an erection during sexual activity. Considerable difficulty in maintaining an erection until the completion of sexual activity. A marked decrease in erectile rigidity. Case Study Treatment Plan
Differential Diagnosis: Differential Diagnoses that are important to consider are Non-sexual Mental Disorders, Normal erectile functions, and substance/medication use. According to the DSM-5, a leading cause of erectile dysfunction in adults is depression. It is worth noting that this could be a leading factor as to why Nestor is experiencing this dysfunction. Many men experience high expectations to perform, leading them to have issues with erectile dysfunction. The DSM-5 includes consideration of normal erectile function in males with excessive expectations. Substance and Medication Usage should also be observed and understood. Nestor is on a relatively new medication for his high blood pressure, and there is a linkage between blood pressure medication and erectile dysfunction. Potential Legal Considerations Associated with the Client, Situation, or Diagnosis: Potential legal considerations that need to be accounted for are those dealing with pornography. Ensure that these images or videos being observed are not of minors. Also, we need to take into consideration HIPAA guidelines. Goals Short-Term Goals to Address the Diagnosis: 1. Attend support groups weekly 2. Communicate with his wife more about his presenting problems 3. Try to lower porn usage to a few times a week 4. Try cutting back on how many days a week Nestor drinks. Instead of drinking 3-4 times a week, try to do three days. Then slowly cutting back as time goes on. 5. Enrolling in a league that involves his hobbies Long-Term Goals to Address the Diagnosis: 1.Implementing porn blockers on all his electronic devices in his home. 2. More control over his drinking by only drinking occasionally or during celebrations. Treatment Plan Strategies to Promote Optimal Sexual Functioning: Ways to promote optimal sexual function is to understand Nestor's past sexual experiences and his current ones. In many relationships, there is usually a mismatch in eroticism. Meaning that what provides Nestor with arousal might not do the same for his wife, so figuring out what each other likes is essential. Also, learning more about the medication that Nestor is on can guide his clinician in understanding if his sexual dysfunction is correlated to his prescription. Figuring out Nestor's alcohol tendencies is another mechanism that can be implemented to promote optimal sexual functioning. According to the DSM-5, substance abuse could play an intricate role in a person's sexual dysfunctions. Evidence-Based Treatment Interventions to Support Strategies: Sensate focus exercises are a popular, proven beneficial treatment. Instead of leaning heavily on genital pleasure, it helps with cognitively remapping what is pleasurable to the individual. In the exercises, one member of the couple plays the
“giving” role, and the other person plays the “getting” role. The giving partner is instructed to massage or fondle the other, and the getting partner must communicate to the giver what is most pleasurable (Hyde, 2019). Cognitive Behavioral Therapy is another proven treatment that works in Sex Therapy. The clinician will help with restructuring what a person's thought process once was toward his presenting issues. In one form of cognitive restructuring, the therapist challenges the client’s negative attitudes (Hyde, 2019).
