Week 5 Assignment Template Diagnosis and Goal Setting Use this template to develop theory-based interventions for the two clients (one adolescent and one child client) you selected to complete this assignment. Upload a copy of your completed template to the Week 5 assignment area. Provide the information as outlined below: Client Name: Edward Client Age: twelve years old Item Description Determine an appropriate DSM-5 diagnoses. Edward presents with Oppositional Defiant Disorder. This was not an easy diagnosis to come up with. It was tricky to narrow in on defiant, angry, and vindictive behavior in an adolescent. Could it have just been normal adolescent behavior or something more? However, because Edwards’s parents stated that his behavior happened overnight, it seems like a cause for concern. Edwards’s parents said that since he came back from winter break, he disrupts class, makes noises, dances in halls, mocks his teachers, mocks his sister, and when she cries, he does it even more. According to Lin et al. (2019) Data indicated that parental emotion dysregulation was related to child ODD symptoms in the home and depressive symptoms indirectly through harsh parenting practices and child emotion regulation. There is not enough information to infer that this could be a leading cause of Edwards presenting symptoms but research shows that ODD can perpetuate from parental emotion dysregulation. This would be essential to remember as the therapeutic relationship evolves. According to the DSM-5, Oppositional Defiant Disorder 313.81 (F91.3), A pattern of angry/irritable mood, argumentative/defiant behavior, or vindictiveness lasting at least six months is evidenced by at least four symptoms from any of the following categories and exhibited during interaction with at least one individual who is not a sibling. According to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V), ODD consists of three dimensions: irritability and anger, vindictiveness, and argumentative behavior, Children with ODD struggle with forming friendships and have problems in school (Eskander, 2020). 1
Week 5 Assignment Template Item Description Develop a minimum of two appropriate long- term goals. 1. Significantly reduce the intensity and frequency of vindictive and defiant behavior toward family, classmates, and teachers. 2. Resolve the conflict that is underlying Edward’s behavior. Develop a minimum of two short-term goals for each identified long-term goal. 1. Identify the feelings or objects of the defiant behaviors and vindictiveness and the reasons behind it. 2. Edward should be able to articulate behaviors that are acceptable towards family, classmates, and teachers. Identify a minimum of three current or potential legal issues that may impact the client’s treatment in counseling. Edward is a minor and because he is only 12 there are many legal considerations that a counselor needs to heade. Legal jurisdictions typically have special laws governing minors, and it is important that counselors be familiar with state laws (Wrape & McGinn, 2019). Especially because a counselor has a responsibility to report and to warn. (Sori & Hecker, 2015) 1. All mental health professionals are mandatory reporters. It is their duty to report when they suspect child abuse or neglect. It is a tricky situation to be in but failure to report breaches legal and ethical standards of mental health workers. 2. Privacy is a huge legal and ethical consideration that must be taken into consideration when working with minors. Legally, parents of minors typically make all privacy-related decisions for them (Sori & Hecker, 2015). Decisions about protecting children’s privacy and confidentiality in counselling must be examined from both legal and ethical lenses (Sori & Hecker, 2015). 3. Duty to warn, since Edward has shown some behaviors that could escalate and put his classmates, family, and teachers at risk it is imperative that the counselor brings this to the attention of the appriorate individuals effected. 2
Week 5 Assignment Template Item Description Identify a minimum of three current or potential ethical issues that may impact the client’s treatment in counseling. Edward is twelve years old so there are quite a few ethical issues that a counselor needs to take into consideration. Section B of the ACA codes of ethics highlights the significance of honoring a clients confidentiality, being ethically sound in their disclosure, and making sure that they are accurately and appropriately disclosing information to the capacity that the client can understand. 1. Code B.1.a, counselors maintain awareness and sensitivity regarding cultural meanings of confidentiality and privacy (ACA, 2014). 2. Code B.1.c, counselors protect the confidential information of prospective and current clients, counselors disclose information only with appropriate consent or with soung elgal or ethical justification (ACA, 2014). The foundation on which counselling is built lies in the ethical obligation to keep information learned in counselling private from others. (Sori & Hecker, 2015). 3. Code 3. B.5.a, when counseling minor clients or adults client who lack the capacity to give voluntary informed consent counselors protect the confidentiality of information received (ACA, 2014). 3
