Case Study of Kimberly Pt 1 1 Case Study of Kimberly, Part 1 Maha Haroon Capella University PSY6210: Intro to Psychopathology Dr. Burgard December 3 rd , 2023
Case Study of Kimberly Pt 1 2 Case Summary In this case study, we are presented with Kimberly, a grungy/goth, 17-year-old, female who lives with her successful, well-polished parents, in an upper-class area. Kimberly also has a sister, who is 4 years older than her and attends an Ivy League School. For a certain amount of time, Kimberly’s mom has noticed a concerning drop in her weight (25 pounds), and explains that she has developed an eating disorder, "smokes like a chimney," is physically and verbally abusive, hoards strange objects, has burns on her arms, has disengaged socially, and has dramatic mood swings. Her moms reasoning for the eating disorder, is from Kimberly’s older sister, as she had an eating disorder in her first year of college but attended therapy and successfully recovered from it.
Case Study of Kimberly Pt 1 3 Clinical Impression Based off this new information in Kimberly’s case, the clinical implications are stated below: Clinical Diagnosis: 296.80 (F31.9) Unspecified Bipolar and Related Disorder 307.50 (F50.9) Unspecified Feeding or eating disorder 305.1 (F17.200) Tobacco Use Disorder, Severe Other Factors: V61.20 (Z62.820) Parent-Child Relational Problem V62.3 (Z55.9) Academic or Educational Problem V62.9 (Z60.9) Unspecified Problem Related to Social Environment V15.59 (Z91.5) Personal History of Self Harm Recommendations Based on the information for Kimberly, I would recommend a visit to the family doctor to check on her eating disorder. She would also need to go through extensive counseling, as well as incorporate psychotherapy. This will benefit her, as it could identify some of the things that she is experiencing. Kimberly’s parents should also allow her to have a one-on-one session alone with the therapist, to see if they can find more answers to her behaviors/outburst. Kimberly can benefit from CBT or DBT, which can allow her to process her outbursts, and her self-harming behaviors, and find out why she’s feeling and acting this way.

Case Study of Kimberly Pt 1 4 Questions & Additional Information Question : What diagnostic possibilities does Kimberly's case present? Response: For the main diagnoses, I believe that Kimberly's case presents an Unspecified Bipolar and Related Disorder, Unspecified Feeding or eating disorder, and Tobacco Use Disorder, Severe. For the other factors that are playing a role in Kimberly's behavior, I believe that she is having a Parent-Child Relational Problem due to a Problem Related to her Social Environment; she is also having Academic or Educational Problems, as well as Personal History of Self Harm. Question : What evidence in the case history presents these possibilities for you? Response : Kimberly presents herself differently than her parents, and the area she lives in. She has 11+piercings, while her parents are proper, and well adjusted, as well as her sister attending and Ivy league university. This social environment factor plays a role in her academic + parental relationship. Her mother describes her mood swings as dramatic and has specified an eating disorder due to her sudden weight + her sister having it in the past. She is told that she can “smoke like a chimney”, and has burns mark on her skin, suggesting self-harm attempts. Question: What kind of questions you might ask to evaluate each diagnostic possibility? You must consider at least two, but no more than three, diagnostic possibilities, and develop a series of questions to interview for each possibility. Response: Unspecified Feeding or eating disorder: 1: Have you always thought about losing weight?
Case Study of Kimberly Pt 1 5 2: What made you want to lose weight quickly? 3: Do you feel like you must lose weight to fit in a crowd? Is anyone making loose the weight? Tobacco Use Disorder, Severe: 1: When did you start smoking? 2: Does smoking give you any relief, if so from what? 3: Do you willingly smoke, or only doing it because of friends? Unspecified Problem Related to Social Environment: 1: What makes you feel so different from your parents and your neighbors? 2: Do your friends provide you with more comfort, if so, in what ways? 3: Do you feel pressured to be perfect because of your parents and sisters? Question: What possible answers would incline you toward or away from each of your various possibilities? Response: I would speak to Kimberly without her mother’s presence. I would ask her if anything specific has happened to her at any point in time. If she explains that some specific event has triggered these symptoms, I will reevaluate the diagnosis to either a neurocognitive or personality disorder. Question: What further diagnostic evaluation is needed to clarify Kimberly's diagnosis? Response: Having her speak to a therapist/psychotherapist alone, so she can speak without having her mother’s presence. I would also do some cognitive/personality assessment to
Case Study of Kimberly Pt 1 6 determine if there is any other diagnosis that can be done. Maybe investigate the family history more, to see if anyone else has gone through this.
