Case Study of Bill 2 Case Summary In this case study, Bill, a middle-aged Caucasian male with two grown children, both father and mother have deceased. Bill still grieves the loss of his father, and states that his relationship with his mother was of “approval seeking from her”. Bill’s father was an alcoholic, and his mother is described as “a strong, critical and judgmental woman”. Bill graduated with a from a prestigious college, went on to serve in the military where a “vindictive” superior officer gave him poor performance evaluations which ended his career. He reports that he has lack of motivation for work and feelings of “emptiness” have been occurring for the last 2-3 months, he also states that he has been experiencing thoughts of “suicide” frequently and has lost “joy”. Bill had said he is on “Prozac, and other substances” and is questioning the validity of using it He is suffering from guilt, and low self-esteem and fear of rejection from his family; he has stated that he has difficulty with Psychotherapy due to the chronic nature of his problems, and his fear of “alienating people who are close to him
Case Study of Bill 3 Clinical Impression Based off Bill’s case, the clinical implications are stated below: 296.89 (F31.81) Bipolar II Disorder, current episode depressed, severe, with melancholic features 305.00 (F10.10) Alcohol Use Disorder, Mild Some other leading factors of his behavior are: V62.29 (Z56.9) Problem related to employment. V61.10 (Z63.0) Relationship distress with intimate partner V61.20 (Z62.820) Parent/Child relationship problems Recommendations Using the information Bill has provided, I would recommend Bill the CBT (Cognitive Behavior Therapy) route, in which he can express his internal thoughts and feelings, especially the sense of alienating people who are close to him. Another strong recommendation would also be for him to attend rehab for a month where he can relearn social skills, and learn coping skills, that can benefit him and his ability to be with his family. Questions & Additional Information Question: What diagnostic possibilities does Bill’s case present? Response : it could be possible Bill is expressing symptoms related to bipolar disorder. Case Study of Bill 4 Response : The most prominent symptoms Bill has stated were “inflated self-esteem” and “grandiosity”. Question : What kind of questions you might ask to evaluate each diagnostic possibility? You must consider at least two but no more than three-diagnostic possibilities and develop a series of questions to interview for each possibility. Response : I would ask Bill if he has experienced any mood swings, and if there is noticeable change in behavior that can be considered unusual. Another question I would ask is if he is experiencing “flight of ideas” or “thoughts racing”. Question : What possible answers would lead you toward or away from each of your possibilities?
