NAME ___________________________ Homework #2 – Major Depression While watching the video of Steve, a man suffering from Major Depressive Disorder (MDD), gather evidence to support a diagnosis. The DSM-5 criteria for MDD are listed below. Provide examples of how Steve meets all the criteria. Also, identify a biological, psychological, social, AND sociocultural contributing factor to Steve’s MDD. Background information (gender, race, age, marital status, treatment history, medication, onset of symptoms, reason for seeing treatment): Steven, Male, White Cognitive therapy Depression Tomazopan (sleep) Prozac (depression) Another antidepressant (could not hear the name) “I find it hard to see a future in anything” Always exhausted no matter what “Whats the point, whats the point in doing anything” “it actually takes over your life” Doesn’t have the willpower or motivation to do anything Mom committed suicide when he was ten Difficult to trust anyone Brings himself to “something hes not” to make people like him Previously married, currently separated One day fine, next day depressed Cannot cope with commitment Feels he doesn’t deserve to be happy Cant build and hold onto relationships Diagnosed three years ago Looks back on basically his whole life and he suffered acutely Passive thoughts about not wanting to be alive Feels like a burden on everyone

Five or more symptoms that have been present during the same 2-week period and represent a change from previous functioning; at least one of the symptoms is either (1) depressed mood or (2) loss of interest or pleasure. a. How long has Steve been experiencing symptoms of depression (Longer than 2 weeks?) When he looks back on his life, Steve states that he has been experiencing depression for as long as he can remember and that it has been affecting him since he was young. b. Give examples of how Steve meets AT LEAST FIVE of the criteria under A. 1. A1, states he feels depressed every day and it is a daily think he has to deal with 2. A6, says that everything he does makes him feel tired, even if he has gotten good sleep 3. A7, states multiple times throughout the video that he feels like a burden on everyone and that there is no point in living because he feels as though he has no purpose 4. A8, cannot commit to anything. Feels as though when he is depressed he needs to be clingy to things and cling on to everything he can, but on days he feels better he wants to rid himself of that entirely and rid other people of dealing with it 5. A9, Stated he thinks about how it would be better to be dead every day, also states how he thinks about killing himself and contemplating suicide every day “I think depression will kill me” B. The symptoms cause clinically significant distress or impairment in social, occupational, or other important areas of functioning. (Describe several ways Steve’s life has been negatively impacted by his depression.) C. The episode is not attributable to the physiological effects of a substance or to another medical condition. (Write a statement about whether or not there is evidence that substance use or health concerns are causing the depressed symptoms.)
D. The disturbance is not better explained by schizoaffective disorder, schizophrenia, schizophreniform disorder, delusional disorder, or other specified and unspecified schizophrenia spectrum and other psychotic disorders. (Write a statement about ruling out other possible mental health conditions). E. There has never been a manic episode or a hypomanic episode. (Write a statement to address this criteria) . Write a conclusion statement that includes the number of the diagnosis, the full name of the diagnosis, and the severity (see pg. 162). (in partial-remission, moderate, Steve is a 30-year-old Caucasian male suffering from Moderate Persistent Depressive Disorder (300.4(F34.1)) in partial remission with persistent major depressive episodes and was early onset. When looking back on his life Steve remembers suffering from depression beginning in his teenage years and has been suffering from it ever since then. He is currently on two antidepressants (Prozac and -I could not understand the other name-) as well as a sleep medication (Temazepam). He is currently in cognitive therapy and takes daily videos to talk about how he was feeling that day. When talking of his depression, Steve says “I find it hard to find a future in anything” as well as feeling things such as “…what’s the point? What’s the point in anything?” (Criteria A7). He also goes into talking about how he feels tired constantly every day. He states that no matter what he does, even simple daily tasks such as hygiene or getting out of bed make him feel extremely fatigued mentally and physically. (Criteria A6). When going over his daily life, Steve talks of feeling depressed nearly every day of the week. There were mentions of feeling worthless and hopeless. When talking about this, he became emotional and began tearing up as well as crying (Criteria A1). Steve has lost interest in doing most things (Criteria A2), however, he finds going out and sitting in nature in areas that his mother also enjoyed brings him feelings of peace, as well as makes him appreciate the beauty of the natural world. He talks of being not motivated to do anything when speaking about his interest lost. Steve struggles with decisiveness when it comes to relationships with other people. He refers to this when talking about his worry about commitment when it came to his past relationship with a woman named Victoria. They were previously together for two years but are currently separated. When asked about this relationship, he began talking about how he feels like he cannot cope with the pressure of commitment. He mentions feeling as though he can’t build nor hold on to relationships. When his ex-wife was asked about Steve and his depression, she talks about how she feels that Steve feels like he does not deserve to be happy. He also talks about how when he is not depressed, he wants to completely rid himself of being dependent on others and wants to diminish his depressed self, but when he is depressed, he is very clingy and only wanted to depend on his Victoria. (Criteria A8) (Criteria B). Steve later mentions experiencing passive thoughts about thinking he was better off dead, and that the world would be better without him as he sees himself as a burden. He also mentions dealing with thoughts of suicide every day and goes on to say “I think that depression will kill me” (Criteria A9). Steve did not mention to have every experienced any manic or hypomanic episodes. He was diagnosed with Major Depressive Disorder three years prior to this interview. The depressive
