Description of Field Experience Activity: During this portion of my field experience on October 1, 2023, I focused on chronic disease management, specifically in the context of caring for a patient with Alzheimer's disease. Here is a detailed breakdown of the interactions that demonstrate the nurse's role in promoting the sphere of care:

1:30 PM - 2:30 PM (1 hour): I visited a patient with Alzheimer's disease who lived at home with her daughter. My primary role was to provide care that aligns with palliative care principles to enhance her quality of life while managing her chronic condition:  Assessments and Medication Administration: I conducted thorough assessments to monitor the progression of the disease and administered prescribed medications to help manage her symptoms and slow down the disease's progression.  Education: I engaged in educational activities by providing information and guidance to the patient's daughter. I explained the importance of medication compliance, proper nutrition, and strategies to improve the patient's daily life. Education is essential in chronic disease management to empower family members and caregivers.  Collaboration: Recognizing the complexity of Alzheimer's disease, I collaborated with the patient's primary care physician through phone communication. We discussed the patient's current condition, medication effectiveness, and potential adjustments to the treatment plan. This collaboration ensures that the patient receives comprehensive care and that her medications are optimized for her specific needs. Sphere of care This field experience activity directly relates to the sphere of Chronic Disease Management. The patient's Alzheimer's disease is a chronic condition that requires ongoing assessment, medication management, and education for both the patient and her family. The nurse's role in this context is essential for enhancing the patient's quality of life and managing the chronic disease effectively.
