Promoting Comfort, Personal Hygiene & Bed Making Healing 3 Assignment 1 Submitted to: Daryl Lagayan Submitted by: Cherilyn Doctor Using the “7-steps” framework for decision making by the HCA, describe the step-by-step approach you would take to handle the following clinical scenario: 1. Confirm Instructions 2. Gather information 3. Perform Activity 4. Observe responses 5. Report 6. Record 7. Evaluate Eleanor is an 87-year-old resident with mild cognitive impairment. She weighs about 90 lbs and is 5’3”. Eleanor has severe osteoporosis, is incontinent of urine but wears Depends, and her right knee is very sore, and she is having trouble walking. You are assigned to provide Eleanor with a bath and get her ready for the day. You enter Eleanor’s room to say good morning and she is huddled under the covers in her bed which is wet. You advise her that you are there to help her with her bath, but she says “No, it is too cold, and my knee hurts too much, I don’t want to have a bath!” Using the problem-solving approach above, what are you going to do? How will you convince Eleanor to take a bath when she is too cold? What can you do regarding the fact that her knee is sore, and she is having trouble walking? How will you deal with her wet bed? What will you report at the end of your shift? Write what your specific report would look like? 1. Confirm Instructions o Acknowledge the task at hand: Provide Eleanor with a bath and get her ready for the day. 2. Gather Information o Eleanor is 87 years old with mild cognitive impairment. o She weighs 90 lbs and is 5’3”. o She has severe osteoporosis. o She is incontinent of urine, but wears Depends. o Her right knee is very sore, and she's having trouble walking. o She's huddled under wet covers in her bed and is resistant to the bath due to feeling cold and having knee pain.
3. Perform Activity o Approach Eleanor calmly and respectfully. o Acknowledge her concerns about feeling cold and knee pain. o Explain the importance of maintaining hygiene for her well-being. 4. Observe Responses o Pay attention to Eleanor's nonverbal cues and expressions while discussing the bath. o Be attentive to any signs of discomfort or distress. 5. Report o Document Eleanor's resistance to taking a bath and her reasons for refusing. o Note her body language and any visible signs of discomfort. o Document any observations about her wet bed. 6. Record o Detail the conversation with Eleanor about the bath. o Note any attempts made to address her concerns and encourage her cooperation. 7. Evaluate o Reflect on the situation and the effectiveness of the approach used. o Consider alternative strategies to address Eleanor's concerns if the situation arises again. To convince Eleanor to take a bath when she is feeling cold and her knee hurts: Offer to adjust the room temperature to make it warmer. Use warm towels or blankets to keep her warm during and after the bath. Propose to wash her in stages, allowing her to rest her knee and regain comfort between steps. Regarding her sore knee and difficulty walking: Ask if she prefers a seated bath or if she wants assistance with specific tasks to minimize strain on her knee. Offer gentle support while helping her move and reassure her that you'll be cautious about her sore knee. Dealing with her wet bed: Express understanding about the wet bed and assure her that you'll address it after helping her freshen up. Prioritize her comfort by changing the bed linens promptly.
Explanation: End of shift report: "I provided care for Eleanor, an 87-year-old resident with mild cognitive impairment, severe osteoporosis, and mobility challenges. Eleanor's right knee was sore, making it difficult for her to walk comfortably. She was resistant to taking a bath due to feeling cold and knee pain. I engaged in a gentle conversation with her, addressing her concerns and suggesting adjustments such as warmer room temperature and breaks to alleviate her knee discomfort. Eventually, Eleanor agreed to a bath in stages, and I ensured her comfort throughout the process. Additionally, Eleanor's bed was wet, and I promptly changed the linens after attending to her needs. I have documented our interactions, her responses, and the strategies employed for future reference and continuous improvement."
