Briefly summarize the situation.
The General Hospital is in the process of switching over to electronic health records. They were
already aware of 5 years of duplicate health records in the master patient index so they hired a
consultant firm to review the data and help cleanup the master patient index by combining
duplicate health records. The hospital began training their patient registration staff, which
includes admissions, emergency department, outpatient services, on proper search strategies,
questions to ask the patient, the importance of a unit health record to help prevent any further
duplicates of patient’s health records. Once the staff were properly trained, the consultants began
the cleanup process and provided the General Hospital with documentation showing which
admissions clerks created duplicate health records, along with which department created
duplicate health records.

What function is being addressed?
The hospital is addressing issues with duplicate health records in the master patient index, that
they need to fix and prevent.
What role can the consultants play in the MPI cleanup?
The consultants can review the duplicate health records and merge them when necessary. They
can provide documentation that shows which health records were and were not duplicated, as
well as which admissions clerks created duplicate health records and which departments created
duplicated health records.
Why did the facility find itself in this situation and what could they have done to prevent
it?
The facility was not properly training their staff and monitor their staff’s paperwork to ensure
accuracy. Training their staff and performing audits will help prevent this from happening again.

Discuss your current career plans within the health information profession.
My current career plan within the health information profession is medical billing and coding. I
want to be able to work from home. I am aware that when first starting out, you will need to
work at the actual facility until you meet the accuracy and quantity needs for the company and
that can take time.
What skills do you think you already possess?
I have previous experience within the healthcare field that I believe will be very beneficial to me.
I have always been very good at keeping calm and finding a solution to a problem. I have an eye
for detail and pick up on things that others may have missed. I know that working in the health
information profession can be a challenge at times, especially with how quickly things change
like new regulations, and new coding systems, and diagnostic codes. I can adapt quickly to new
situations.
What do you need to work on in order to successfully pursue the desired career?
For me to successfully pursue medical billing and coding I will need to continue to learn medical
terminology and coding. I also plan on, once I have gained more knowledge, applying for an
internship so I am able to see what they do and hopefully get the hands-on experience that will
help prepare me for my new career. I need to work on getting more knowledge and
understanding of the laws, regulations, and coding software. I know there is more to learn and I
am excited to start this journey.
