Alice, a 17-year-old, came into the primary care clinic looking very emaciated with a body weight of 85 pounds and a BMI of 14.4. After carefully questioning her regarding her eating habits, she confessed to avoiding high-fat, high-calorie foods, eating tiny quantities of low-calorie foods, and admitting to weighing several times a day because she always feels fat. Also, Alice states exercising three times per day to burn calories. She appeared to have little muscle tone with severe muscle weakness. Her extremities were lacking circulation and had a blue appearance to her fingers and toes. Her vital signs showed low blood pressure and a slow, weak heart rate. Her lab results revealed iron deficiency anemia, electrolyte imbalance, and dehydration. Several factors that influenced Alice’s condition include chemical imbalances in her brain, past trauma regarding food, negative social attitudes about food from her friends, and anxiety towards life. To treat Alice’s eating disorder first, we must establish a healthy diet: eating three meals daily with adequate protein, vegetables, carbohydrates, and fats while consulting a nutritional coach to educate Alice on eating health. In coordination with nutritional consulting, Alice needs to attend psychotherapy to change her negative thoughts regarding food while working through her fears and feelings. Adding antidepressant medication can improve her anxiety symptoms of food as a negative. Getting her family involved in her care will give her someone to talk with and learn from at home while keeping her accountable with her medication and healthy eating. Reference Anorexia nervosa .
