Goal Development Worksheet 1. Take the Self-Assessment (in the Week 2 Learning Resources of the online classroom) in which you rate your readiness on advanced practice behaviors. Summarize your Self-Assessment results in the box below. 2. Based on your Self-Assessment ratings, identify two practice behaviors that you would like to work on during your internship from each of the nine competencies . 3. Transfer these prioritized practice behaviors to the worksheet below. 4. Create a SMART goal for each of the identified practice behaviors using the following model: Student will insert practice behavior by date , as evidenced by measurement . Note: Remember that goals do not necessarily have to span the entire internship. You can write shorter-term goals to be accomplished within 1–2 weeks and then add onto or replace them later. Summary of Self-Assessment Summarize the results of your Self-Assessment here, including the priority areas for learning and skill demonstration in your internship. Competencies 1-5 I scored a 4 for all categories except for #16 I scored myself with a 3 because I know about agency and government policies but I could expand further on my knowledge and gain more confidence by doing so. Competencies 6 I scored a 4 for each category and for Competency 7 the Individual category. However, Families I scored a 2, Group, Organization, & Communities I scored a 3. Competency 8- Individual was a 4, Groups & Organizations were a 3, and Families and Communities were a 2. Competency 9- Individuals was a 4 and the rest of the categories were a 2. In short I gained a lot of experience working with individuals in my first internship and for this internship I need to focus on working with groups, communities, & organizations.
Competency/Practice Behavior Goal Example: Competency #1 Practices the ethical and appropriate use of technology. Nicole will practice ethical and appropriate use of technology by 5.12.22 , as evidenced by completing Walden’s ethical training module . Example: Competency #8 Utilizes appropriate resources and services for clients.
