FUNCTIONAL BEHAVIORAL ASSESSMENT REPORT Student name: Nicolas Meeting Date: Student ID #: Disability: Grade: Home School: Case Manager: Min Oh Team Members Present at Meeting: Min Oh / BCBA Name/Title Name/Title Name/Title Name/Title Name/Title Name/Title Student History and Background (include a description of the student’s behavioral strengths, relevant academic placements and skills, diagnosis, etc.) : Nicolas is an 6-year-old male who has been identified as a person with an intellectual disability, and a blood-clotting disease called Von Willebrand (VW) disease. He has been brought in for consultation for his VW disease as well as nose picking behavior. His intellectual disability is moderate, with good language skills but limited expressive communication. His nose is currently healing and he should not be allowed to touch or pick it. If any bleeding occurs, you have been instructed to insert a special cotton swab and call 911 as a precaution. From interviews with parents, it was learned that this behavior problem is relatively new. Until this past Spring, Nicolas did not display this behavior. Previous interventions attempted to help manage this behavior problem have included the following: Verbally reprimanding him (“no picking”), while physically blocking his nose. This will only temporarily stop the nose picking, and he will soon start again. They would also try to distract him through play, but will soon start picking again after they stop playing. Typically, before sneezing, he will get a tissue and make a sort of “fake sneeze.” Description of Behavior Targeted for Reduction (include an operational definition and baseline data/current status of behavior) : Nose Picking: Any attempt to place hand or finger on any part of his nose. Nicolas regularly picks at his nose, and it is a problem because he picks often enough to have complications due to his blood clotting disease. Functional Behavioral Assessment Methods: (list all methods utilized during assessment) Precursor functional analysis (PFA), since allowing nose picking is risky and he has a reliable precursor (fake sneeze). The functions run are control, attention, and ignore (with blocking), to see why the nose picking is happening. 1) He is not stimulated (bored). 2) Parents stop playing with him. 3) Parents take away his toys. Consequence(s) (include a description of the relevant events that follow the behavior) : After all relevant antecedents, Nicolas proceeds to start picking his nose. Other Potential Variables (include a description of any other factors/variables that may affect the behavior, such as medication, weather, diet, sleep, substance abuse, attendance, social factors, etc.) : Nicolas started nose picking in the Spring, he is taking blood clotting medications, and has an intellectual disability. Prior Interventions (include a description of the behavioral interventions that have been implemented in the past, including the date(s) of implementation, length of intervention, the impact of the intervention on the student’s behavior, etc. Attach data summary, if appropriate) : A PFA has been conducted with control, attention, and ignore conditions. The function has been determined to be automatic and attention. Summary Statement/Hypothesis of Behavioral Function (describe the team’s hypothesis of the relationship between the behavior and the environment in which it occurs – what function is this behavior serving for the student? What is the student trying to get? What is he/she trying to avoid?) : The student Nicolas seems to be picking his nose for a combination of automatic / sensory and attention-seeking reasons. Nicolas seems to be stimulated by the nose picking, and can also do it when he wishes to continue playing with parents. Treatment Recommendations (Provide recommendations for prevention of the target behavior, replacement skills/behavior(s) to be taught, reinforcements for positive behaviors, etc.) : For prevention of target behavior, whenever Nicolas exhibits his reliable precursor (fake sneezing), either block his attempt to pick his nose, or provide attention to him, as relevant. Nicolas can also be taught a replacement for nose picking, something else that could also provide sensory stimulation.
