Assessment Name: Nicole Date: August 31 st , 2022 Please complete each section in narrative style Identifying data Nicole is a pregnant white 15 year old that identifies as female and identifies as pansexual who is currently a resident of Big Hearts. Presenting issue Nicole is a 15 year old female and pansexual whose presenting problem is having anxiety due to her fears of uncertainly with her future. Nicole reported a history of depression and anxiety, sexual abuse by her father since the age of 5 years old, prostitution and the use of meth, which she stopped consuming due to her pregnancy. Nicole expressed that she would like to keep the baby, but would like to put the baby up for adoption as she is unsure she can provide what the baby needs and deserves. In addition, her partner Joel would oftentimes assault Nicole when he was using. She is not sure she is in love with Joel because she has developed feelings for another resident in Big Hearts, but her current partner Joel is incarcerated. Nicole has told him that she will keep writing/visiting him and will be there when he is released from jail. Nicole described herself as someone who has a difficult time making personal decisions as she is fearful that the decision she made will bring bad consequences. In addition, she added that she is the type of person who says whatever she thinks the other person wants to hear. Strengths and resiliency Nicole reported that she wants to obtain her GED and also earn a college degree. Through observation it was observed that Nicole was resilient in deciding that she no longer wanted to live in a home where she was abused by her father. Past psychiatric history Nicole has a history of depression and anxiety. She experiences panic attacks where she freezes and paralyzes to the point where she is unable to think or concentrate on problem solving. Nicole stated that her panic comes from fears of uncertainty-like her future at the moment. She vividly mentioned that she “freaks out (has full blown panic attacks, crying, feeling desperate, feeling like her life is going no-where or her situation is hopeless”. Nicole has not had treatment for her depression and anxiety. Substance use history (Could be “none” or a description of past difficulties to include whether or not there was prior treatment) Nicole has frequently used meth but when she found out that she was pregnant she stopped. She reported no additional use of other substances. Developmental history
Nicole reported that her father sexually molested her at the age of 5 until the age of 12. Nicole ran away from her home when the sexual abuse and yelling and level of violence in her family became unbearable. Biological/Physical factors Nicole is in general good health, although she has a history of depression and anxiety. Nicole has not consulted with a psychologist about her symptoms, thus she has not been medically diagnosed. Nicole expressed that her anxiety comes from fears of uncertainty. She did not express coping techniques at this time as her last coping method was her frequent use of meth. Nicole mentioned that she ran away from her family of origin at age 12 because her father was an alcoholic and her mother was scared to leave and help her. It was noted that Nicole’s father sexually molested her since the age of 5. Nicole did not report any current medication or physical disabilities. Psychosocial factors Nicole is currently in a relationship with who she believes is the father of her baby. The father of the baby, Joel, is 21 years old and is currently in jail due to drug dealing. Joel was sentenced to 20 years in jail with the possibility of parole in 15 years. Joel begged Nicole to continue their relationship while he was in jail as he wanted to be there for the baby and Nicole. Nicole reported that prior to Joel being prosecuted, Joel assaulted Nicole when he was using. Nicole mentioned that she feels safe at Big Hearts. Nicole used to live on the streets, prostituting herself for money and shelter until she met Joel who provided her with food. Nicole did not report if she currently still has a relationship with her parents. Coping Mechanisms The previous coping mechanism Nicole utilized was meth, but due to her pregnancy, she stopped her frequent use of meth. She did not report any coping mechanisms at this time. Cultural identity Nicole described herself as someone who says whatever they think the other person wants to hear as Nicole expressed that she does not like confrontation. She added that she has a hard time making decisions and sticking by the ones she’s agreed to is because she doesn’t want to admit she could make a bad decision. She did not describe a cultural practice but did mention that she believes in God. Spiritual identity Nicole reported that she believes that God will provide for her and expressed that God has always provided for her. She added that she does not go to church nor identify with a specific religion.

She has a history of depression and anxiety. She is seeking assistance in coping with her anxiety as she has fears of uncertainty- like her future at the moment. She described that her anxiety is getting out of control and “freaks out (has full blown panic attacks, crying, feeling desperate, feeling like her life is going no-where or her situation is hopeless).” She was sexually molested by her alcoholic father from ages 5-12 years old. As a result, Ms. Nicole ran away from her house and lived on the streets. She added that she was prostituting herself for money and shelter. She met her current partner, Joel, while she was prostituting. Ms. Nicole believes that Joel is the father of her baby, but she currently has feelings for a resident at the shelter Big Hearts. She promised to Joel that she will write to him and wait for him while Joel completed his time in jail as Joel expressed that he would like to be in the baby’s life. She mentioned that she would like to keep the baby but wants to place the baby up for adoption. Nicole appears to be highly motivated as she wants to obtain her GED and also earn a college degree. She believes in God but does not identify herself with a religion.
