Identify the case study you chose for this discussion. Case Study 1-1 Lilly Goodman Define the presenting problem as presented in the case study. Lilly has been a go getter all of her life. she has spent many years seeking new learning opportunities. She was not encouraged to finish school growing up and ended up dropping out of school after only obtaining a 5th grade education. Lilly did not let this stop her she started cleaning houses at the age of 12 and continued to do so until her body was no longer able to handle it. She is now in the hospital following two strokes. The strokes coupled with crippling arthritis. Lilly has her independence and now requires assistance with a lot of her ADLs. Lilly is struggling to adjust to what is her new normal. She is likely not safe to return home on her own and will either need caregivers or to reside in an assisted living facility. She is not able to accept this and is her new reality. She is looking at having to become a resident of a nursing home, figure out what to do with her house, depend on others for assistance, and deal with debilitating illness. In addition to dealing with these new identities Lilly is also having to adjust to being surrounded by others who are struggling with debilitating illnesses and relying on the state to fund her care when she has always been so proud of her ability to pay her own way in life (Lecroy, 2014). Provide the appropriate DSM-5-TR diagnosis for the case study you chose including the appropriate ICD-9-CM code. In my opinion an appropriate DSM-5-TR diagnosis for Lilly would be adjustment disorder 309.9 and anxiety disorder F41.9 (APA, 2013) Using Gehart's guidelines for writing measurable goals, develop and write at least two measurable goals for the case study you chose. 1. Lilly will attend physical therapy as prescribed for three months to increase her independence with ADL's 2. Lilly will attend 3 social gatherings (i.e meals in the common area, happy hour, puzzle time, craft sessions, etc) per week for 3 months to increase socialization and relationship building.
