Rutgers University Office of Field Education Process Recording Template – Format # 1 Individuals, Families, Groups Your name: Giorgina Kirner 1. Date and Brief Description of Contact On January 26 th , 2024, a sixteen-year-old client contacted us through our crisis text line, threatening suicide because her 17-year-old boyfriend dumped her after she disclosed that she was pregnant for him. 2. Goals for the intervention or meeting : My primary goal was to ensure the physical wellbeing of the client, render the emotional support relevant to their crisis, propose constructive coping mechanisms to alleviate their emotional distress, assess whether they have any support system, and encourage them to seek professional help. 3. Plans: I planned to build rapport and trust with my client by leading with empathy and demonstrating genuine concern and understanding of their situation. Building a rapport with my client encouraged them to share their struggles openly. My next step was to assess the client’s level of risk using an evidence-based risk assessment. I also planned to assess the client's protective factors to identify coping mechanisms and accessible support systems they can rely on to alleviate the situation as they await professional help. Finally, I planned to direct the client to where they can access professional help. 4. Content of the interaction :
Client : I can’t bear this pain anymore. I want to die. Me : It sounds like you are hurting deeply, and I am sorry you feel that way. Can you tell me more about it? I would like to listen and help where I can. Client: My boyfriend just dumped me because I am pregnant. I don’t know what to do. My parents are going to be so disappointed in me. Me: I understand that your boyfriend’s reaction to the pregnancy news may be deeply upsetting, and thoughts of motherhood at your age may understandably feel overwhelming. I also understand that you may be nervous about how your parents will react to the news. I will walk with you, and we will get you the help you need. You are not alone. 5. Your observations of yourself and the client or other people you met with : I observed that the client was emotionally distressed due to the sudden relationship breakdown. She was also nervous that her parents might react the same way as her ex- boyfriend, leaving her to bear the responsibility of raising her child alone. Although I felt the urge to take action right away to ensure that the client was safe and unable to harm themselves, I restrained myself and instead chose to approach the matter calmly so as not to trigger the client to pull back from confiding in me. I also hesitated to notify the client that I was legally obligated to notify the authorities about her situation. 6. Your thoughts and feelings : I was sad and worried for the client because she was a minor facing the possibility of dealing with a life-altering situation alone. I also feared for the wellbeing of the unborn child. Moreover, I struggled to notify the client that I was legally obligated to report our
conversation to authorities because I feared they would panic and pull back. However, I attained this goal by communicating with the client with compassion and calmness, which brought positive results. 7. The social work competencies you demonstrated I demonstrated supportive counseling skills and ethical responsibility by balancing client confidentiality and my obligation to report cases involving danger to the client’s safety to relevant authorities. I also demonstrated active listening skills, which allowed me to approach the client's crisis with empathy and understanding. 8. Your evaluation/analysis of the interaction : I met the client’s goals by creating a safe, conducive environment that allowed them to share their predicament openly. I also worked with the client to identify coping mechanisms to alleviate their situation. Additionally, I was able to link them to places where they could get professional help. I plan for future interventions to rely on more evidence-based research to address client crises. This will provide solutions that respond adequately to modern-day clients' evolving needs. 9. Questions for supervision What is the most effective evidence-based risk assessment for minors presenting with suicidal ideation?
