7. If the patient is now diagnosed with prostate cancer and has been found to have tumour in less than half of one lobe, what is his T stage? What other information do you need for further staging? The T stage is T2a. PSA levels, gleason score, and a biopsy (+the clinical stage found) 8.The patient’s Gleason score is 3. What is the Gleason score? How is it determined? Why is it significant? Gleason score is a scoring system which is taken into account by giving two Gleason scores. The first one is for the most common patterns that we see and the second is the second most common pattern. You combine both scores by adding to get the final Gleason pattern score. It is determined by removing the cancer cells surgically, biopsy, or removal of the prostate. Since Gleason score is 3 the cancer growth is slow. Significance: The gleason score rates the aggressiveness of the cancer. 9. What is this patient’s overall stage? What are the patient’s treatment options? What are the pros and cons of each option? T2a N0 M0 Surgery and brachytherapy are the two treatment options available - with surgery having a higher impotence rate and also a higher chance of urinary incontinence. In this case, surgery can be offered for T2 ( generally in good health ). The forms of cancer include: - Radical retropubic prostatectomy ( most common ) + nerve sparing - Preservation of sexual function - Laparoscopic radical prostatectomy - Nerve sparing is possible - Radical perineal prostatectomy - no tissue spare Some long term complications with surgery include impotence (40-76%) and urinary incontinence (6-35%). Another option is LDR permanent brachytherapy — 5 year survival rate - Con: Impotence ( 20% ) + urinary incontinence (10-15%)
