QI proposal topic : Evaluation of the effectiveness of Tele-Pharmacy Services and multifaceted interventions on medication adherence among Older Adults with Chronic Conditions Justification of the importance of this topic : In 2020, a study led by Tecklenborg, et al revealed that most geriatric patients’ adherence to their medication reduced by their second visit to the health facility. This is an important topic given that medication adherence is directly linked to effective treatment and healing. Researchers have established that some of the factors influencing medication adherence in elderly patients can be mitigated using technology and multifaceted theory driven interventions (Emadi et al., 2022). It has also been established that these factors are not linear and may include lack of knowledge, complex medication regimen, memory difficulty and economic difficulty (Tecklenborg et al., 2020). Given the diverse factors that influence geriatric patients not to adhere to medication, a multifaceted approach would be beneficial for this population. Technology has proven to be very impactful in revolutionizing the delivery of care (Emadi et al., 2022). With remote devices accessible to patients and their health service providers, it will be easier than ever to deliver constant reminders to geriatric patients to minimize the risk of them failing to adhere to medication. According to the findings by Pan et al., (2023) some of the factors influencing medication adhehernece in the geriatric population are behavior based. Thus multifaceted theory driven interventions can be effective especially when based on the Health Belief Model. PICOT Question: In geriatric patients (P), does the implementation of technology-based interventions and multifaceted theory-driven strategies (I) compared to standard care (C) improve medication adherence (O) within the first two visits to the health facility (T)? _______ (P), _______ (I), _______ (C), _______ (O), _______ (T). Benchmark 2: Achieve a minimum of 70% patient satisfaction rate, measured through surveys assessing user experience with tele-pharmacy services and multifaceted interventions . Patients appreciate receiving help with their medication adherence challenges. A study by Pan et al (2023) established that telepharmacy can increase patient satisfaction by providing remote regular reminders to promote adherence. Benchmark 3: Achieve a 35% reduction in emergency room visits due to adverse events caused by medication non-adherence among geriatric patients in the tele- pharmacy services and multifaceted interventions . Telepharmacy and multifaceted approaches will likely minimize the occurrence of adverse health outcomes due to poor medication adherence subsequently reducing the emergency room visits (Tecklenborg et al., 2020). Benchmark 4: 15% reduction in hospitalization rates linked to chronic conditions among participants receiving tele-pharmacy services and multifaceted interventions. This benchmark reflects the potential impact of improved medication adherence on health outcomes and healthcare resource utilization (Tecklenborg et al., 2020).
