CHCCCS006 Facilitate individual service planning and delivery Task 2 Contribute to the planning of services for at least three people by: How will I liaise, prior to the planning session, with the person who conducts the needs assessment, so the results can help inform the planning process I would liaise with the assessors in my 3 clients to establish the needs, identified goals and objectives. Kenneth – needs transportation and managing tasks around the home. Cynthia – needs assistance with re-engaging in activities and connection with care team, Mei – needs assistance with tasks at home, practicing her mental health exercises and re-engaging in activities. How will I identify the person’s current life stage? Kenneth – is in the late adulthood stage, he will be experiencing diminishing sight, hearing, memory loss, slower processing, using less vocabulary, more susceptible to illness and be less effective at reasoning. He could be experiencing empty nest syndrome and reduced ability to think clearly. Cynthia – is in young adulthood and in the prime of her life Mei – is in middle adulthood, her physical fitness will be waning, she will be starting to show signs of aging and want to be part of the community. How will I identify the physical and psychological factors that will influence service delivery? Kenneth – I can identify his physical and psychological factors by consulting with his daughter, I can ask her details about what works best for her dad, personality questions, what his idiosyncrasies are, how best to encourage him and best form of communication and soothing techniques. I could look at his diagnosis report and any other reports or case notes from other health professionals. Cynthia – I can consult her health professionals, cultural leaders and family or friends Mei – I can consult her medical professionals, psychologist, cultural leaders, previous case notes and family How will I discuss with the client or their guardian :? o the purpose and desired outcomes of the planning process o I would advise all of my clients that the purpose of the plan is to make an individualised approach to how I can best support their identified needs, goals and objectives. The desired outcome is achievable goals and services best matched for their needs. o who should be included in the planning process o The clients are the most important people to be involved in the planning process, I can also invite family, friends, carers, health professionals, cultural representatives, financial advocates, relevant service providers.
o different service options that will meet client needs o Using the person centred approach which allows for client preferences, opportunity and choice, I would do the following: o Kenneth – I could offer him our own volunteer-based community transport but I would also offer him a couple more options to see which one best suited his needs and then let him make the choice. I would also offer my services for in-home assistance and also give him a good alternative to give him freedom of choice. o Cynthia – I would give her a variety of options for her art class including the one at the centre here, I would present the different options with their locations, times and dates and let her pick the best one for her. o Mei – I would give Mei some options for in home assistance including my own and let her decide which one she would like and I would also give her a list of activites in the area, including a local Chinese group if she preferred to be in her cultural group. o the client’ readiness to adopt and adhere to an individualised plan o I would ask questions around my client’s needs and goals to ascertain readiness to make a plan, this would include looking out for a positive outlook, readiness to engage, ability to cope with and understand the plan and a sense of accountability. o the most appropriate service options o Kenneth – I can see the most appropriate options are based on his needs, these are transportation and assistance in the house. If he has not already registered, I will register him for an assessment with My Aged Care so he will have access to subsidised services. o Cynthia – Cynthia’s most appropriate option will be based on her location preference, time, day and content of class. o Mei – Mei’s service options will be based on her need for assistance in the house, what type of activity she would like to re-engage in and a support worker to assist her with her mental health exercises. I would check if she is registered with the NDIS. collecting, recording and disseminating information about the plan to stakeholders and organise the practicalities so the process I would advise the clients that we need their consent to collect, disclose and store their information, I would present a consent form for them to sign and I would explain that we will keep their information private and confidential and only share their information with people who need to know such as service providers and health professionals.
