CHCCCS006 Facilitate individual service planning and delivery Task 1 Communicate effectively with at least three people to: How will I identify client needs? – ask relevant questions, actively listen, build trust based relationships and goodwill. I would ask relevant questions with Kenneth, I would address his feelings of isolation resulting from travelling alone on transport, I would ask Cynthia what interests her and listen to her preferences, I would also ask questions relating to locations, preferred time and dates, I would ask Mei how her pain management was going, how she was coping with daily tasks and whether she was able to practice her mental health strategies as well as discussing her interests. I would build trust by being non judgemental, creating a safe space and keeping confidentiality, instil confidence that they will be safe in their activities and show genuine concern and empathy in a warm manner. I would also ensure I showed up when promised. How will I respect the multi-faceted needs (financial, physical, emotional, cultural) of clients and their families? As a Community Service worker, I can recognise and respect the multi- faceted needs of Kenneth, Cynthia and Mei by actively listening to identify needs, questioning to find out their additional needs, confirming and clarifying requests to ensure correctness, asking open questions to elaborate on their needs and not making assumptions. I would respect their culture and personal space, provide empathy, show patience and consideration and use open and friendly body language whilst observing theirs How will I collect information that will support planning of suitable services? I would collect information for Kenneth from his family member for background and personal information, Cynthia’s interpreter may be able to clarify more details for her and I could engage Cynthia’s mental health practioner. I could use other stakeholder or service providers information that may be available for all 3 clients. How will I comply with privacy legislation and workplace processes to protect client information eg password protect computers; ensure that unauthorised personnel do not have access? I would actively comply with privacy legislation by ensuring that my workstation is locked when I am away from my desk, ensure all paper files are kept seculrey locked away, ensure my computer has anti-virus software installed and access codes required for locating online files. I would ensure any unwanted paperwork that had been scanned was shredded and not just thrown into a bin or recycling.
How will I gain written consent from clients to release personal information? I would ensure I obtained consent by having Kennet, Mei and Cynthia sign our organisations consent form and upload this into our database. How will I provide clear and current information about service delivery? For Kenneth, I can provide detailed information about Community Transport, I can explain the different companies, their service frequency, the area they cover, costs associated and booking times. I can also give him a current flier on each of these services. For Cynthia, I can give her information on the art class that is most suited to her time and location preferences and another option to decide from. With Mei, I can give her information on anxiety support groups in her area with contact details and frequency, I can also give her the details of a local Chinese group who engage in regular activities and have a current flier, I can also give her the information for an NDIS provider that would be great for assisting her in her house with daily tasks. How will I collaborate with other service providers if services must be provided by external agencies? I will send a referral to other service providers for the services that my community centre cannot offer. I can offer a warm handover for Kenneth, May and Cynthia so they feel safe and supported and can also trust the service provider. How can I ensure the interests and rights of clients are protected and they can contribute to decision-making? I would provide my clients with choices of services and ample information to allow them to choose the service that they think is the best fit for them. I will advise them that ultimately the services are their choice and they have the right to decide which one is best for them.
