Obtains needed equipment and supplies: replacement cannula or tracheostomy care kit, hydrogen peroxide, sterile saline, sterile gloves, clean gloves, goggles, mask or mask and face shield, tracheostomy holder if the patient does not use ties, and suction catheter kit. 2. Suctions the tracheostomy following the steps in Skill 28-07. 3. Follows “Initial Implementation Steps,” and dons clean gloves. To Replace a Disposable Inner Cannula 4. Removes inner cannula after first unlocking it, and disposes of it in the biohazard trash. Also, removes old tracheostomy dressing from beneath the faceplate. 5. Inserts the new inner cannula by starting to insert at the 9 o’clock position, then turning gently to the 6 o’clock position. 6. Disposes of inner cannula and used dressing in biohazard trash. 7. Goes to steps 8–13. To Clean and Replace a Reusable Cannula 4. Sets up tracheostomy kit contents on sterile field and prepares basins with peroxide and saline solution in one and saline solution in others. 5. Removes the inner cannula from the tracheostomy and drops it into the basin compartment containing the peroxide and saline. Removes old tracheostomy dressing and disposes of it in biohazard trash. 6. Removes clean gloves and sanitizes hands. 7. Dons sterile gloves and arranges sterile field. Uses sterile small bottle brush to clean the inside and outside of the inner cannula with the peroxide/saline solution. Rinses inner cannula in a basin compartment containing only sterile saline. Burton Funds of Nsg Care 3e Skill 28-08 7. Dries inner cannula with sterile 4 × 4 on the outside and both pipe cleaners together to dry the inside. Inserts the cleaned inner cannula. 8. Uses saline-moistened cotton swabs to clean around the stoma. 9. Changes tracheostomy ties or holder, ensuring that one finger fits under the ties or Velcro holder. Has an assistant hold the faceplate in place while changing ties, or if working along, does not remove old ties or holder until new ties or holder are secured in place. 10. Places a clean tracheostomy dressing beneath the faceplate with open ends pointing towards the patient’s chin. 11. Ensures that the tracheostomy collar is in place over the tracheostomy if ordered and that oxygen is being delivered per order. 12. Follows the “Ending Steps.” 13.
