Introduction: A child who refuses food often or eats the same food over and over again is referred to as a picky eater. Picky eating mostly peaks in the toddler and preschool years.
Explanation of Solution
Person G, aged three, is a well-groomed boy. His growth is normal, but he does not talk. Person G walks and moves well, but he prefers to play alone. Favorite food of Person G includes macaroni, cheese, and white bread without crusts, mashed potatoes, honeycomb cereals, and crackers. Person G throws food like hamburgers, fruits, most vegetables, any food combinations that he does not like.
Person G’s mother takes him to a pediatrician due to his picky appetite, and the doctor told there is nothing to worry. His mother is very much frustrated due to his eating habits and feels difficult to take him out. Person G is not much interested to eat food when others eat, and he is satisfied in drinking juices. Person G has been referred to a speech therapy, and his medical history shows that he has three ear infections, and there is no major illness.
Person G’s nutrition assessment shows that he is taking around 1350 calories per day, and his diet includes excessive amount of vitamins C and B with adequate protein.
The signs that Person G’s feeding problem is related to his speech are as follows:
- Person G only prefers to consume easily digestible and soft-textured foods like macaroni, cheese, and crackers. These types of soft texture and mild taste food are relative more toward a child closer to one year of age, whereas Person G is almost three.
- Person G dislikes eating solid food that required more effort such as chewing and prefers liquid foods that are easy to ingest.
- A child by the age of three will be able to voice out the preferences in food. Since, Person G is speech impaired he finds it difficult to say his preferences and this will cause frustration. This frustration is shown as tantrums.

The dermatologist explained that treatment options at this stage are limited. The drugs available for chemotherapy have only temporary effects, and surgery is not effective for melanoma at this stage. The dermatologist recommended that Bruce consider entering a clinical trial that was testing a DNA vaccine for melanoma treatment. These vaccines deliver DNA encoding a gene expressed by the cancer cells to the immune system. This primes the immune system to respond by producing large quantities of antibodies that destroy melanoma cells wherever they occur in the body. A clinical trial using one such DNA vaccine was being conducted at a nearby medical center, and Bruce decided to participate. At the study clinic, Bruce learned that he would be in a Phase Ill trial, comparing the DNA vaccine against the standard treatment, which is chemotherapy, and that he would be randomly assigned to receive either the DNA vaccine or the chemotherapy.
