2 Field Agency Submission I have to identify a viable agency from my community, which I am going to use my agency again and submit the information on the Lead Agency Form in Meditrek. I will have access to this form once my field application is accepted. In reviewing placements, the field office looks for diverse opportunities to ensure students have a rich and varied learning opportunity. This includes but is not limited to a diversity of culture (communication, feedback, leadership, policies, procedures, macro systems, etc.), to allow students to compare/contrast between organizational structure and engagement; new learning opportunities that can enhance student learning; working with a diverse population; etc. If a student is requesting to waive a requirement through a petition process, the student will need to clearly document that they have thoroughly exhausted all possible placements in their area. T he deadline for submitting my Lead Agency form for field quarter summer 2024 is the application on March 11, 2024. This is the date your four-part application packet is due in Meditrek. You can submit your packet earlier than this date; however, the review may be delayed if you’re submitting your application early. The lead agency form deadline is March 27, 2024. This is the date a prospective agency should be submitted in Meditrek by. The internship doesn’t need to be secured by then, but you should contact the agency prior to submitting their information. The field placement deadline is May 8, 2024. This is the final deadline to have an approved internship. Your Field Experience Coordinator partners with you to ensure sites can provide an appropriate Field Experience as outlined by the CSWE guidelines. Site paperwork and task conversations must be completed prior to this date.
