1 Adverse Childhood Experiences can alter the lifespan of an individual who has been exposed to growing up in homes involving violence, abuse, neglect, mental health or substance abuse issues, parental separation, or incarceration. In this the Hernandez family, there are several components that match ACEs. As a social worker, I would make a definitive plan for this family based on the identified ACEs. Starting with, their parenting styles. In the Social Work Case Study, Elena, and Juan both expressed that they feel very lentinan when disciplining their children. Since Elena was raised with harsh punishment, for instance getting beat with a belt and Juan Jr. was raised holding books in his hand, their discipline mechanisms are essentially all they know as parents. The identified ACEs in this family dynamic include substance abuse, neglect, incarceration, and unstable income. With these ACEs, I would first have to assess the trauma of the children. Per the CDC, I would use the ACE questionnaire and Pediatric ACEs and Related Life Events Screener (PEARLS). The social worker from ACS mandated the family to attend weekly family sessions and complete a parenting group at their local community mental health agency. She also suggested they receive education about child development because she believed they had unrealistic expectations of how children at that developmental stage should behave. Other than the mandated suggestions the worker has implemented, like all screenings, a trauma screening such as the ACEs questionnaire is meant to be used only to indicate the need (or lack thereof) for further assessment and does need constitute a thorough assessment in-and-of itself. It is also important to remember that no screening tool represents an exhaustive list of traumas that a client may have experienced and so it is essential to continue to explore the potential presence and impact of other traumas as you work with a client progressively. I would
2 administer one ACE questionnaire per child, and then also use the Family Health History Questionnaire, and the Health Appraisal Questionnaire. T he Family Health History and Health Appraisal questionnaires were used to collect information on child abuse and neglect, household challenges, and other socio-behavioral factors in the original CDC-Kaiser ACE Study. After the results are measured, depending on the score of all questionnaires, I would then base my recommendations off of each test. Adding up the number of ACEs a person reports having been exposed to (or a parent reports a child has been exposed to) is termed “cumulative scoring.” This type of scoring challenges traditional notions about what is a more versus less severe traumatic experience or event. However, the ACEs dose-response effect consistently emerges in research irrespective of the specific ACEs involved. It is a lot more complicated than it may seem when trying to administer the functions of the results.
