First Level I Fieldwork Experience Day 1 Client: Marcus Marcus identified the occupation domain of work as being a meaningful occupation to him. In Marcus’s case, he does not have trouble at work, but he has trouble getting motivated to leave the house for work. He stated he will be ready for work, but he will “lollygag” on the way out the door. I would suggest adding the Hamilton Depression Rating Scale to Marcus’s assessment. Marcus reported believing he has depression but has not been diagnosed. The HDRS would give the OT practitioner insight into the severity of the client’s perceived depression and the practitioner could adapt interventions to help Marcus cope with the depression as well as their diagnosis of anxiety. The OT practitioner cannot diagnosis clients, but we can provide education on topics and collaborate with the interprofessional team. Reference: Hamilton M. A rating scale for depression. A Peer Support Specialist is an individual who has experienced mental health challenges and can provide support to the clients who are currently going through treatment. I believe Peer Support Specialists are a vital part of recovery and treatment for current mental health clients. The COTA and Peer Support Specialist may collaborate to create activities that the clients will truly enjoy. The Peer Support Specialist may also help the COTA create a stronger rapport with their clients.
The attributes that a Peer Support Specialist demonstrates include empathy, active listening, hope, cultural humility, collaboration, and feedback skills. These attributes are similar to the attributes that my classmates and I have. When working with my classmates, we collaborate well together for a common purpose. In our efforts to effectively collaborate, we practice active listening and feedback skills. We also practice giving each other hope and positive encouragement throughout our class and program. As an aspiring COTA, I would assume the role of relaying observations and insights regarding the clients’ achievements, challenges, and progress. I would use effective communication to collaborate with the Peer Support Specialist to create activities that could support the client and specialist’s goals. First Level I Fieldwork Experience Day 3 The next intervention activity I would select for Marcus is mindfulness meditation practice. This meditation practice would be guided by a meditation app or website. I believe this would be a “just right” progressive activity for Marcus because during the KAWA model activity, he enjoyed listening to calming music. The supplies needed for this activity are as follows: comfortable space (chair, yoga mat, pillows, etc), an application or video for meditation, phone or computer, and an audio speaker for clarity of guided meditation. Guided meditation would be beneficial for Marcus and his treatment journey because mindfulness is beneficial for mental health. Practicing mindfulness can produce calming breaths and a decrease in heart rate which is beneficial for anxiety. I would like to use a guided meditation for Marcus that follows the same ideology of the KAWA model. A meditation app or
video that focuses on the flow of life while promoting relaxation through river sounds would be ideal. “Leaves on a River” guided meditation would be the video I would choose for Marcus.
