Final Project Case Study and Cognitive Behavior Therapy Intervention Jim is a 14-year-old Caucasian male, and he lives with his mother and father. They have no known religious affiliations. Jim's father attends AA meetings to maintain his seven years of sobriety. Jim was doing well in school as he was on the A-B Honor Roll. As for childhood friends, he does not seem to be affiliated with them anymore. He lost his sister due to a car accident five months ago. The family dynamics have changed since the accident. Jim no longer gets good grades in school, and he is hanging out with “goth” friends according to his father. He has been isolated in his room and has not been talking to his family. The teachers are showing concern for his academics, and social and emotional development. He has been disengaging from his regular activities. His recent hobbies including drinking alcohol and huffing are concerning his parents. They took the next step to get Jim’s help and sought a counselor. The newer developments that Jim is going through happened within the five months since his sister’s death. His isolation and lack of connections to his parents are due to his mother and father’s lack of interest in addressing the real issue. His mother is saying he is going through a teenage phase and his father does not take the initiative to speak on the issue as well. As his symptoms seem to result in grief. The mental issue presented is adjustment disorders with the disturbance of conduct, acute F43.24. Counseling Intervention Jim is suffering from a traumatic event that changed his outward way of living. He changed his attitude and dynamics in relationships and level of effort in school. The counseling intervention that suits a case like this one would be Cognitive Behavioral Therapy (CBT). The article states “For a patient with AD, the goals are to reduce emotional and behavioral distress 2
Final Project and improve functioning” (Alvarado, 2022, p.3). This treatment option is suitable for a case like Jim’s because he is going through an emotional and behavioral level of distress due to the loss of his sister and the lack of communication support within the family. Research Question Is Cognitive Behavioral Therapy (CBT) effective in treating young teens and children who are diagnosed with an adjustment disorder? Within the diagnosis of adjustment disorder, there is a lot to consider in a person’s well-being. The case with Jim is a young teen whose life has shifted in all aspects of his daily life. His social and emotional development has decreased from how he was before. Cognitive Behavioral Therapy allows further understanding of the status in which he is in and alters his thinking. Search Strategy The research studies that were selected focus on the elements of adjustment disorder with disturbance of conduct, acute F43.24. I use the keywords search in the Walden Database library to narrow my search in reference to the main research. Unfortunately, there is little research on adjustment disorder; with that, I gathered the main symptoms affecting the client. The research articles that were founded were peer-reviewed. This is focused specifically on children who have gone through trauma-related incidents and how the caregivers play a role in the treatment. It dives into the functional impacts on the lives of the children such as daily living, relationships, academics, etc. This study is a mixed study and it focused not just on the child’s trauma but the caregivers. The caregiver’s history of trauma affects their child and indicates how they associated and relate to their child. They were assessing the child and caregiver in the therapy session and there were questionnaires they had to fill out. This research connects to the case study because the client is going through trauma and his caregivers are his parents. This study uncovers the underlying emotions of the child to which the caregiver’s history is determined. The participants of this research are traumatized, children and nonoffending caregivers. This study was taken on youth located in Delaware at different mental health agencies. Both caregiver and child were in receiving sessions of (TF-CBT) and each was seeing the same therapist in 30–45-minute sessions out of fifteen. The child baseline symptoms checklist resulted in older kids showing more caregiver blame during the sessions. According to the caregiver’s trauma history, it was resulted that many of them experience multiple traumas before in relieving during TF-CBT. Also, they were avoidant. As for the correlations between the child and caregiver, there was avoidance and blame of the caregiver from the child displaying negative emotions. The outcome of this study showed a great impact on how a caregiver during TF-CBT sessions when supporting their child there's better assisting and understanding of the child's 4
Final Project trauma. The case study benefits from this research because of the behaviors and experiences its displays. Jim and his parents are at the beginning stages of this traumatic experience. In the short time that his sister has passed, the parents have not spoken and found coping strategies to help their child. The social and emotional developments of the youth in the study were shown to be impacted according to the TF-CBT. The youth were lacking self-regulation and had low executive function and trauma-related psychopathology. The services where the study had taken place were in the Northeast region of the USA. The criteria had to be one including the following sexual abuse, witnessing domestic violence, traumatic bereavement, etc. This study was done through the parents’ support reporting the functions of the youth through the BASC Parent- reported Executive function scale. The results showed that TF-CBT improve the overall quality there was a pre, mid, and post-treatment assessment there was an improvement in the following assessments. This study was a quantitative study. The youth in this study show a higher improvement in overall treatment during TF-CBT. the case study shows similarities to this research study in the way how Jim’s social, emotional, and functional impairment. Everyday use of therapy focuses on the central issues weighing on a person who needs consolation and guidance. Therapy as usual beaches talking and finding solutions seeing a therapist once a week. In this study, this research is comparing TF-CBT and therapy as usual for trauma-exposed children. The study was approved by the Regional Committee for Medical and Health Research Ethics and there was written consent for the participation of children and parents. The ages of the children were between 10 to 18 years of age and were referred to outpatient clinics. There was a checklist that describes the types of experiences the children had. The children who were eligible by the CPSS were all assigned randomly to either TF-CBT or TAU. There were even therapists who were assigned to TF-CBT or TAU. The TF dash CBT was short term and there were 12 to 15 sessions both child and parent were in the session and separate. There were many talks on psychoeducation relaxation and interpersonal. Final Project children showed a significant reduction and traumatic symptoms. There was a reduction in mental health problems, behavior, and emotional problems in the TAU group. This was a mixed study and upon the session, there were also questionnaires that the children have taken for mental health. This article, although it didn't specifically address adjustment disorder, addresses the main symptoms that go along with Jim’s case. In this study, not just the traumatic experiences are being treated with CBT but it's being compared to TAU. This indicates that children with traumatic experiences benefit more from TF-CBT therapy than regular TAU therapy. Evidence-based Practice and Application to case study The findings of these articles provide information on cognitive behavior therapy and how it can help adolescents who are dealing with Adjustment disorder with disturbance of conduct F43.24 acute. Although the articles were not specifically focused on adjustment disorder, they provided the main symptom similarities and what the case study focuses on. In all three articles, the intervention provided using CBT shows significant results in the cases of traumatic children. For my chosen client, for example, Jim is dealing with isolation and lacking emotional and social interaction do the recent traumatic events. As a counselor, Jim is running CBT sessions to decrease levels of isolation and symptoms related to his grief. He is showing awareness and openness in finding solutions to better his state. Yet at the same time, we are at the beginning and assessing the client and helping form coping skills take time. Treatment plan (CMHC & MCFC) A treatment plan for Jim who is dealing with adjustment disorder with disturbance of conduct F 43. 24, acute. Is looking to first assess the level of concern we are looking to provide a TF-CBT-specific intervention. This of course with parents’ involvement alongside Jim we are addressing the relationship between him and his parents. How are they dealing with the tragic 7
Final Project loss of a family member? It’s important to address who the adolescent is around and who are their caregivers. The treatment is at least six months or more depending on the sessions needed. There will be pre-and post-questionnaire for both parents and Jim addressing the mood, experiences, and how they are coping. The communication between Jim and his parents is guided by the counselor. This is allowing space for the client and caregiver to understand their positions. The goal is to assure the lowering of negative emotions and the increase of positive emotions and mental growth. Getting Jim to a point in his life where he is able to regain normal function and live a better life. Conclusion In this research paper, it was concluded that adolescents with adjustment disorder with disturbance of conduct F 43. 24, acute. Or with similar symptoms resulting from experiencing traumatic events. In the case study of Jim who is dealing with a disorder and would benefit from Cognitive Behavioral Therapy specifically trauma-focused. How Jim’s personal state has altered his emotional and social being affected in all aspects of his life. The discoveries of the articles that were used to determine the outcome of this intervention granted sufficient evidence-based treatments for the population in which Jim is. This research has prepared me to dive deep into looking for relative and effective treatment options to case deal with important issues such as in the case of Jim.
