Case Summary Molly, a 29-year-old single woman, appears with a variety of symptoms, including sadness, sleep issues, difficulty concentrating, and a 15-pound weight loss over two months. She's been having these symptoms for two and a half years, ever since she lost her work at a high-pressure computer start-up. Molly's depression was prompted by an intimate affair with a coworker, which resulted in humiliation and threats of firing. She lost the respect of her partner and coworkers, and by six weeks, she was too miserable to function. She was fired. Molly has taken numerous similar positions but has been unable to overcome her reservations about her luck. She has returned home with her supportive parents and has started smoking marijuana for relaxation and numbness. She smokes at least two marijuana cigarettes every day and drinks two to five glasses of white wine in the evening to sleep. Molly has no medical issues except for trouble ovulation and irregular menstruation. She has never had an accident, ailments, surgery, or medicine that has worsened her health. Molly also mentioned that her mother took her to her first session with her therapist. She is embarrassed to be in the clinic and her current situation because she stated that she never needed to see someone about it. Clinical Impression (Diagnosis) Primary Diagnoses: 296.23 Major Depressive Disorder, Severe, With Anxious Distress 305.60 ( F14.11) Stimulant Related Disorders Cocaine 304.20 (F12.20) Cannabis-Related Disorders 303. 20 ( F10.20) Alcohol Use Disorder Other Factors: V61.10(Z63.0) Relationship Distress with Spouse or Intimate Partner V 62.29 (Z56.9) Problem-Related to Employment (job loss) V 60. 9 ( Z59.9) Potential loss of housing due to job loss Recommendations It is recommended that Molly should do the following: Cognitive-Behavioral Therapy: Assists Molly in identifying and understanding her concerns, reorganizing brain thinking, and developing coping mechanisms. Alcoholics Anonymous or Narcotics Anonymous meetings: Helps her overcome her temptation to use alcohol, cocaine, or marijuana. Family Support: Encourages Molly to invite family members to sessions to provide comfort and support. Sleep Study: Determines the underlying reason for sleeping issues that contribute to nocturnal drinking Questions I'd want to look into her family's history of infidelity, which contributed to her depression. Pinpointing the origin of the desire to initiate the affair will help us better grasp Molly's thought process. I'd also like to meet with Molly's parents to
understand their relationship better; it was indicated in the chapter that her mother and father are not pleased with her drug use, yet they continue to send her money.
