CHAPTER 4 DISCUSSIONS and CASE STUDY 1A The Ethics Committee is especially needed when patient autonomy and the law collide. Listen to this video to understand how the Ethics Committee is evolving from settling ethical dilemmas to guiding ethical decisions proactively. https://www.youtube.com/watch?v=edt1F8WU2oo o In what ways do you think the Ethics Committee could settle ethical dilemmas more effectively? The Ethics Committee is to review specific, individual patient-care situations and dilemmas. After the review, the Ethics Committee will provide an opinion about the situation from an ethical point of view. 2A Considering the Ethics Committee Structure, Goals, and Functions, do you think that the structure and goals of the Ethics Committee properly support their functions? Why or why not? Do you think the structure and goals could be modified to improve the functioning of the Ethics Committee? How? 3A Watch this video and consider the ethical considerations in being a guardian. Do you think, after watching this video, that the Ethics Committee could serve well as a patient’s guardian? No, Ethics Committee is an advisory resource. An ethics committee is not a decision maker but a resource that provides advice to help guide others in making wiser decisions when there is no clear best choice. https://www.youtube.com/watch?v=dWD4EHZsNmI Chapter 4 Case Study 1. Please review this video. https://www.youtube.com/watch?v=iPXyz6FBM4U o Did your opinion change about any of the ethical dilemmas through this video? No. o What ethical dilemmas were faced by the guardians during the aftermath of the accident, and what do you think the Ethics Committee would have decided about this case? Why? I think the dilemma faced was not knowing the patient’s wishes and medical miss communication. I don’t believe the doctors were clear enough about her condition to her parents. Her dad claimed “she loved life from her very first breath” so he believed he had to keep her alive until her body would give up. She woke up after
CHAPTER 4 DISCUSSIONS and CASE STUDY 20 years, however her quality of life was declined. I think the ethical committee would have decided that would be best to turn off life support. 2. Now review this video https://www.youtube.com/watch?v=IPHxlXb64EQ o Did your opinion change again after seeing this video? It did make me question a few things, but to me her case is a little different. It wasn’t an accident where she had brain damage, and other physical injuries. She got sick and she was showing signs that she was there, in this case I wouldn’t have given up on her either. o What ethical dilemmas were faced by the guardians with the patient in a coma for 4 years, and how did that turn out? The doctors telling the parents that she wouldn’t wake up and that she would have life long damage, leaving the parents to make a decision that not even the doctors were certain. o What do you think the Ethics Committee would have decided about this case? Why? The ethical committee might would have asked for a second Dr. opinion. It is hard to answer that because on that video it is not clear what was all the diagnosis and exams were saying at the time they were trying to figure it out what was wrong with her. But I like to believe that since Victoria was sick, and the doctors were trying to find the issue to try to cure her, that the committee would have said to keep her alive, at least for a while. 3. Having watched these 2 videos presenting different results than expected, have your opinions changed regarding the use of assisted suicide or withdrawal of life support? Why? To me it depends a lot of the case, the situations in these two videos were very different, on the first video, where the girl had been in an accident and was in coma for 20 years, even now knowing she woke up I still keep my opinion about withdrawing life support. Before making that decision I would wait maybe a year or two to see how things were going, if the patient was showing any brain activity, etc. However in her case, once she woke up, she didn’t have a good quality of life, had to go through surgery and was completely dependent on other people. On the second video, Victoria Arlen becomes ill, she showed brain activity, and was making progress, I would have not chosen to withdraw life support.
