Facilitator Interview Clarisa Hermosillo Northcentral University MFT 5102- Legal, Ethical, and Professional Development in Marriage and Family Therapy Dr. Moncrief October 5, 2023
2 Facilitator Interview I had a meeting in place for October fourth. Unfortunately, Ramona did not put it on her schedule. The rescheduled meeting was held October sixth. The meeting was at 6:30 am. Due to work, and other personal responsibilities the meeting had to be held at that time. Romana has a practice open in Las Vegas, Nevada. She has experience with court keeping, substance use, and domestic violence. She works closely with the court. She mentions most of her clients come from court referrals.  Ramona is a licensed MFT provider and is supervisor for MFT students. Romana teaches her students to have care, and make sure they are putting the client first. For Ramona, being a mental health professional means taking care of her clients and making sure the client is meeting their treatment goals.  Some of the legal challenges Romana faces are related to mandate orders. The probation officer might ask about details on the client's treatment. Romana explains that it is unethical to share client information. The therapist can share information if it is consented by the client ( American Association of Marriage and Family Therapy, 2015). I asked if it’s difficult getting a client that has a mandate order to be compliant during treatment. Ramona explained that sometimes it can be challenging. The client might be defensive and upset having to attend a session. She tries her best to make the client feel welcomed and accepted. The therapist is compassionate and cares about the client’s well-being. She clarifies any misconceptions the client may have. Talking about confidentiality, and the privacy involved in treatment. This follows the ethical code 2.7 ( American Association of Marriage and Family Therapy, 2015). The client will need to provide written consent to share information with the court. If there is an order requested by the court the therapist is responsible to share clinical notes.
3 Some of the advantages working with insurance panels is its a form of self-advertising. Most clients will seek services that are covered through their insurance. Some of the problems that will arise is payments might go through slower and following up with insurance companies. Ramona mentioned when she first started trying to balance clients, and doing the billing for insurance companies can be difficult. Having employees that can handle billing will relieve some stress. Ramona explained that some mental health professionals are more concerned about finances than client care. This will conflict with professional and ethical guidelines. Caring for the client and making sure they are getting high quality treatment.  When discussing case notes it’s important to write down everything that happens during the session. If the client expresses their feelings towards the case manager by using profanity. The therapist might write in the notes the client is upset about seeking treatment. Ramona mentioned since she works closely with the court, she needs to make sure her notes are appropriate for a court hearing. Clinical notes can be misinterpreted depending on who is reading them ( Wilcoxon, Remley, Gladding, 2021, p. 166). Being detailed as possible, and using professional language will decrease the number of misunderstandings.  When terminating a client there’s an ethical procedure that must be followed. In the clinical notes the therapist must provide information prior to terminating. Ramona says it's not very common to terminate a client. There must be inconsistency, and a lack of respect for someone to be terminated. If necessary, the client can be referred a different therapist. As a therapist maintaining professionalism and continuing to support the client’s goals is essential for the therapeutic relationship (Griffin, 2017).  When talking about teletherapy clinicians must follow ethical guidelines. Even though the environment is different, ethical guidelines must be respected. Ramona mentioned she goes

4 over confidentiality during the intake, and the type of environment is appropriate. During the pandemic she found that victims of trauma, and domestic abuse would have a hard time finding a safe environment, and sometimes the perpetrator will be around. The client might not feel comfortable sharing personal information. If privacy is not being respected treatment will not be successful (Lustgarten, Garrison, Sinnard, Flynn, 2020). Romana experiences a lot of different diverse situations. She’s been in practice for many years, and there are new situations that arise. When talking about ethical guidelines and keeping the best interest in the client. Ramona mentions that in most of these situations she will look over ethical guidelines, and sometimes consult with colleagues. She practices the ethical decision model ( Counseling Education, 2015-2021). Sometimes our own values impair our clinical judgment.     The overall experience meeting Ramona was very insightful. She provided a lot of information about career, and personal experience. When discussing client relationships, it was great to hear she teaches her students to care for the client, and to know when it’s appropriate to terminate treatment. Ramona works closely with the court, and she manages to take thorough notes. Writing down client information and being aware that anyone can view them. I was slightly nervous to ask about clients overstepping boundaries and having to address this concern. I think overtime with training and experience it will get easier to address decisions that can be impaired by your morals and values. .
