In this video, I will propose an evidence-based care plan that patients and caregivers can easily follow, which involves all partners in improving patient safety and outcomes. Caitlyn Bergan, the patient in this case, was evaluated in the Vila Health media scenario. An evidence-based care plan is a treatment plan supported by the most up-to-date scientific research evidence. This sort of care plan aims to improve patient outcomes using interventions that have been thoroughly studied and found helpful. To begin, some background information on the patient in this instance is as follows: she is two years old and has been admitted to the hospital twice in the past half year with a pneumonia-related diagnosis. The patient and her family come from a rural community known as McHenry, located further than an hour away from the medical facility. Because of its remoteness, the rural community has limited access to medical facilities. The patient was diagnosed with cystic fibrosis after a more in-depth assessment of the patient's symptoms, vital signs, and test findings. Caitlyn's physician, pediatric caregiver, and respiratory therapist all need to work together to provide the best care possible for Caitlyn, which is complicated because the patient lives far away from the hospital. In this case, the proposed collaborative and evidence- based plan is the use of telemedicine, more especially videoconferencing and electronic health records, to give medical health care services to a patient when they are located afar. According to Jong et al. (2019), telemedicine is feasible since it enhances clinical outcomes in remote healthcare delivery, facilitates interaction between the patient and the physician, and offers cost savings.
3 Caitlyn can receive medical healthcare services remotely for her condition by utilizing videoconferencing and electronic health records. One example of this would be videoconferencing to do consultations with the medical team without needing to go to the hospital physically. This can be very helpful in managing chronic illnesses such as cystic fibrosis and follow-up care. A virtual evaluation of the patient's condition, such as analyzing their symptoms or checking their vital signs, might be done via videoconferencing as part of the plan. This can assist in identifying any alterations in the patient's condition and enable timely treatments if necessary. This will also be utilized to conduct follow-up sessions with the care providers, in which they may review their care plan, evaluate their prescriptions, and address any issues or concerns that they may have. Lastly, the plan calls for using electronic health records (EHRs) to store and exchange the patient's medical data. This information includes the health history, medical reports, and care plans. This can help guarantee that all healthcare practitioners treating the patient have access to the most recent information, which can increase both the quality of care and the coordination of that care. The following illustrates how an evidence-based care model was utilized to create a treatment plan for this particular scenario. The Stetler evidence-based practice model allows for a review of the situation, which in this case was the readmission of the patient with pneumonia, the search for the key findings, the inclusion of the most applicable evidence- based practices for application, and the evaluation of the results (Ewer et al., 2022) that helped identify and establish this care plan. It is evident from the research findings summarized by Adams et al. (2020) that telemedicine can be a valuable tool for enhancing the standard of care and patient outcomes. This is supported by the evidence that seemed most pertinent in the decision-making phase for the treatment plan. These findings are significant when caring for patients with such illnesses because cystic fibrosis and pneumonia may call for continuing management and care coordination, and the patient is from a far area. In addition, remote collaboration can bring benefits and problems to developing and delivering treatment for a patient who suffers from pneumonia and cystic fibrosis. The ability of members of an interdisciplinary team to communicate with one another despite being located in various locations or even time zones is the primary advantage of multidisciplinary teams using remote collaboration. Patients may also get care from a distance through remote collaboration, which may be especially helpful for patients living in rural or underserved areas. Additionally, Ford et al. (2020) suggest that managing and controlling pneumonia through a remote collaboration strategy can enhance patient outcomes, medical systems, and the standard of care. One of the challenges is insufficient access to the technologies required for remote communication. The availability of a diverse range of communication technology helps alleviate this problem's effects. If some team members do not have access to telehealth equipment, they are free to use other available options, such as calls and messages. A further challenge is the timing of remote communication, which frequently cannot be carried out during regular operating hours. In the mentioned scenario, healthcare practitioners resolved the problem by organizing sessions outside normal business hours or exchanging messages by text messaging. In conclusion, the widespread adoption of telemedicine and other communication technology in the healthcare sector has made it possible to establish effective forms of remote collaboration. Numerous studies have shown the benefits of telemedicine, including improved patient outcomes and the value of care. Multidisciplinary teams can provide high- quality care to patients in remote areas, where access to medical services may be restricted, by using the technologies mentioned above and adhering to the most effective procedures and
5 protocols for the various treatments. This is of utmost significance since it contributes to reducing healthcare inequalities and guarantees that all patients, irrespective of where they live, have equitable access to the medical attention they require.
