ABC MEDICAL CENTER FINAL PROJECT PRESENTATION BEVERLY HORSFORD

INTRODUCTION: GOALS Through activities that involve group participation, ABC Medical Center intends to implement a group participation program for a specific group of patients. This program will provide patients with the opportunity to learn and share information about medical challenges and issues that are related to their diagnosis. The patients who would be included in this group would be adults who have been diagnosed with diabetes, hypertension, and congestive heart failure. An important factor that will determine whether this effort is successful is the number of patients who will take part in the program.

INTRODUCTION: EXISTING PROCESS There are multiple departments involved in the current process of data collection and management in the medical center. Some of these departments used paper records, while others used electronic methods. The facility report also stated that patients are given paperwork to take home to fill out and return. After that, the paperwork and any other records that come in hard copy are transcribed into the electronic health record (EHR) system of the hospital. Date of birth, diagnosis code, gender, and a unique patient identification number are some of the data received from patients. However, only one member of staff is responsible for reviewing the data that has been entered into the electronic health record (EHR) to ensure that it is accurate and consistent.
