When it comes to type 2 diabetes there are several risk factors that play a part, some of them are, socioeconomic status, access to healthcare, food environment, cultural and social factors, and health disparities. 2.

Type 2 diabetes can be prevented by living an active lifestyle. There are several diabetes preventions programs today. By attending you’re your yearly checkup and getting blood work drawn providers can tell if you are at risk for diabetes or prediabetes. Once that is determined they can recommend you to a diabetic prevention program or diabetic self-management education and support program (John Hopkins Medicine, 2021). Both groups are beneficial and can give you great insight that could possibly reverse your diabetes. 3.
What outcomes related to diabetes are of most interest to community members?

Some outcomes that are related to diabetes that have drawn interest to the community is prevention and awareness, early detection and screening, management and control along nutrition and diet. When people are diagnosed with type 2 diabetes that want to learn how to manage it, so they don’t go into a crisis and have further complications from it. 4.

The APRN has access to compare states measures of the most recent years and they can determine the strongest and weakest measures by state.
