Skills Concept map: IV piggyback medication Procedure/Device: IV piggyback medication Definition: An IV piggyback is a technique for injecting medication through an intravenous (IV) line. It can also be referred to as an IVPB or secondary infusion. It includes attaching a smaller medicine bag to the main IV line and letting it drip medicine intermittently. The medication that needs to be given is in the secondary bag, and the maintenance fluids or flush are in the primary bag. Who gets this procedure or device and why? It is important to administer many medications slowly via a secondary line to ensure the complete delivery of the prescribed dosage of an IVPB. the piggyback method of I.V. the infusion pump allows for dilution and slower, more precise administration of I.V. medication, in turn decreases the risk associated with rapid infusion (e.g., phlebitis and infiltration resulting from rapid administration of highly concentrated medication) Body System(s) affected: Integumentary Circulatory If there are different types of devices, what are the names and differences? IV piggyback for gravity: The healthcare provider regulates the infusion rate by using a clamp on the IV tubing, which can either speed up or slow down the flow of IV fluids. An IV flow rate for gravity is calculated in gtts/min. IV piggyback for infusion pump: The infusion rate is regulated by an electronic pump to deliver the fluids at the correct rate and volume. Standard Nursing care to monitor the patient and maintain the equipment: Always prepare the medication using the 7 rights to medication administration, whenever medication is pulled from storage Label the IV solution bag at the medication cart with the patient’s name, date, time, medication, dose concentration, and your initials. Calculate IV rate (gravity or IV pump) before going to the bedside Tubings are labeled with date time and when it expires Note when the flush bag expires Change primary and piggyback tubing that remains continuously attached every 72 hrs to minimize breaks in the closed administration

Complications: Symptoms: Nursing Action: Infiltration pain, swelling, and skin that is cool to the touch follow your facility policy and, as a general guideline, discontinue the site and relocate the IV in a new area. apply compress and elevate affected limb Phlebitis redness and tenderness along the vein and can lead to infection if not treated Discontinue IV, relocate new area, warm compress, nonsteroidal anti-inflammatory medication as prescribed. Notify provider Allergic reaction Itchy skin, fever, skin swelling around IV site Administer epinephrine if patient has anaphylaxis and provide oxygen per provider orders, monitor patient and vitals Medications given to slow/quick Headache, tight feeling in chest, irregular pulse, flushed face Discontinue as soon as the mistake has been confirmed. Inform the provider as soon as possible, in order for the next step to be taken to counteract effects of medication administration timing. Monitor patient and vitals
