You are the only clinical assistant in the practice and your morning has been very
hectic. The contaminated instruments have not been processed for the afternoon.
When you finally have a break and get back to the sterilization center, your dirty
instruments are in the ultrasonic, and the trays and paper products are on the
counter.
1. How could this backup have been prevented?
Preventing the Backup: The dental office may have taken a number of actions to stop this
backup,
Streamlined processes, more personnel or training, effective scheduling, and routine
maintenance
2. You have five trays of instruments in the ultrasonic. What can you do?
While those were processing in the ultrasonic, I would prepare more trays with all the necessary
instruments. I would prepare additional trays with all the necessary instruments while those were
being processed in the ultrasonic.
3. What could you have done immediately after performing a procedure to change
this circumstance?
Clean contaminated things right away to avoid delays in instrument processing, and ask
colleagues for help when necessary. Make sure the ultrasound trays are prepared in ten minutes
or less, clean the trays beforehand using disinfectants, and use flash sterilization in case of an
emergency. As assigned, include lab technicians, dentists, and hygienists in the sterilizing
facility.
4. Is there anyone else in the dental office who could help keep this situation under
control? If so, how could you work through this circumstance?
Yes, there's a chance that other dental office employees could assist in maintaining control of the
situation. Like dental hygienist for example.
5. Should you discuss this experience with the dentist?
Although it is ultimately your duty to make sure that your contaminated instruments are
thoroughly pre-cleaned and sterilized, it is crucial that the dentist understand that, if you are the
only clinical assistant scheduled for the day, the turnaround time between patient appointments is
insufficient to allow for the processing of instruments.
