W HAT DO I NEED IN ORDER TO COMPLETE THIS ASSESSMENT ? § Access to your workplace § Access to individualised plans § Access to required equipment outlined in the plan § Workplace assessment and planning tools § 3 skills development records (provided) § 3 client consent forms. W HEN DO I DO THIS TASK ? § You will do this task in your workplace. § Write in the due date as advised by your assessor: _____________________________________ WHAT DO I NEED TO DO IF I GET SOMETHING WRONG ? If your assessor marks any of your answers as incorrect or having insufficient detail, they give you feedback and talk to you about resubmission. You will need to fix the sections that were not correct/sufficient and resubmit. I NSTRUCTIONS : Select three different individuals with a disability within your workplace. For each individual you will need to record evidence of being involved in the skills planning, assessment, skills development and implementation. Skills development planning should be undertaking during a meeting with the client, their family and/or carer. Implementation and review will occur after you have developed a plan with the client during the initial assessment. Use the skills development record provided to document the following information: § Planning and skills assessment Goals Actions When People § Implementation and review Progress Communication Learning opportunities You will also need to answer a number of written questions for each client. Remember to obtain client consent from each client prior to completing this task. Client consent forms have been provided.
