Quality Improvement Project Template Guide ***Use this guide to complete Part 1*** Part 1 of the QI Project asks you to complete the Institute for Healthcare Improvement (IHI) Quality Improvement Project Charter . The charter is an excellent example of how to identify the problem you want to focus on and how to address the problem. But it is limited; the example in the IHI document is helpful but it is also very brief. This Quality Improvement Project Template Guide provides helpful information about how to use the charter and requirements for completing the assignment. Please follow this guide as you complete the template and use the IHI document only as additional information. Use each of the bolded terms in this document to organize your project proposal and create headings. Note: One paragraph is approximately 4-6 sentences. I. Introduction A. QI Project Team Members: identify at least five people who will be on the QI project team. What expertise would be helpful to have on the team? Include professional titles of each team member and explain how these individuals will contribute to the project. B. Project Title: Provide a title for your project. C. Start and End Dates : When does the team propose to begin the project? When will it end – when does the team propose that the solution will be implemented? II . What are we trying to accomplish? What is this project about and why is it important? A. Problem : What is the key problem that your team wants to address? Describe the problem in one paragraph of 4-6 sentences. Be sure to: 1) describe the problem generally – how is this a problem in the U.S. overall? And then, 2) describe the problem in your hospital. Use at least one source that shows this issue is a problem. B. Project Description : Describe briefly how you will carry out the project to address the problem. Include the following: 1) Do other hospitals have a similar problem? What have they done 2) In what unit in the hospital will the project take place? and 3) Provide a proposed high-level outline (i.e., not detailed, one-sentence activities/actions that will occur, include month/year) for the project that lays out how the project will progress. Describe the project in about two paragraphs. Use at least one source to support your description. C. Rationale : Explain why the current process in your hospital is not working and needs to change. How are patient care and patient outcomes affected? How are staff affected? How is the hospital affected, such as financially? Show in this section that something has to change – explain why it is a problem. Describe this rationale in one paragraph. Use at least one source to support your description. D. Expected Benefits : Once the solution has been implemented and change has occurred, how will the unit, hospital, staff, and patients benefit? For example, will the hospital have improved ratings from patients? Will the hospital save money? Will the attrition of nurses decrease? Will patient care and outcomes improve? Describe the benefits in one paragraph. Use at least one source to support the benefits you provide. Note: Don’t talk about the changes/solutions that needs to happen. Focus on what will be the benefits once a change DOES occur. E. Aim/Goal Statement : In one sentence and using SMART goal guidelines (Specific, Measurable, Achievable, Relevant, and Time-bound), state what the end result or the goal you want to achieve. For example, for the topic of improving call light response times, here is a possible goal statement: By December 31, 2024, call light response times by nursing staff in the Medical-Surgical Unit at Amani Hospital will be reduced by an average of three minutes (from the present average of eight to a new average of five minutes) after implementing a three-step solution that includes the implementation of a call light log, using 30-minute active check-ins with each patient, and hiring more certified nursing assistants (CNAs). This comprehensive goal statement meets all of the SMART criteria. It states what the outcome will be, who will accomplish the outcome, how the outcome will be achieved, and when the outcome will be achieved. It can also be used to create the three project outcomes in the next section. Part 1 - QI Project Proposal Template Guide 1
III. How will we know that a change is an improvement to the current process? This section of the project proposal you are creating should focus only on Project Outcome Measures. In this section, identify three ways (i.e., measures, approaches) that the team will use to determine if the goal statement has been reached and whether the solutions applied really did make an improvement. Each project outcome should include each of the SMART goal criteria. Some examples of measures used to determine if an improvement has actually occurred are patient satisfaction surveys of hospitals and the care they received, reduction in staff attrition, staff satisfaction surveys, decrease in hospital debt, reduction in readmissions within 30 days of discharge, statistics gathered that show a decrease in falls or a decrease in patient self-described post-surgical pain experiences. Other measures that rely on qualitative data could include interviewing staff, patients, or family members after the solution has been implemented and find out what they think about the change in their own words. Be creative about the measures you select. Use at least one source to support your description. Below is one outcome measure that is based on the goal statement in Part II and focuses on the goal of reducing average call light response times using one strategy – active one-minute ”check-ins.” Project Outcome One : In order to reduce call light response times by an average of three minutes in the Medical-Surgical Unit, by August, 2024, every patient will receive an active one-minute “check-in” by a CNA or RN every 30 minutes that will include walking into a patient’s room, looking at or asking about Water, Phone, Food, Alarms, & Bathroom (WPFAB) and recording what time each check-in took place on the master check-in board at each nursing station. This outcome measure includes what the primary outcome will eventually be (i.e., the overall outcome in the goal statement), when the approach or strategy will occur, who will accomplish the approach or strategy, how the outcome will be accomplished, and how the strategy or approach will be measured (i.e., times recorded on the master check-in board). IV. How will the QI project start and who will be affected by the project? A. Initial Activities : What will be the first steps in carrying out the project? Include: 1) What data need to be collected about the problem? 2) Who will you interview who has knowledge of the problem? and 3) What type of information do you need from patient or other hospital records? Describe these activities in one paragraph. Use at least one source to support the activities? B. Key Stakeholders : There are stakeholders in every new project. These are people who will be affected by the change that will take place once a project is completed. They could be affected positively or negatively. For example, using the call light example, because CNAs and RNs will be doing the active one-minute check-ins, they will be primary stakeholders that need to be considered. They will need to be trained to complete these check-ins, and the CNAs will need to be supported and overseen by the Nurse Manager on the floor. They could be negatively affected if too few new CNAs are able to be hired. And of course, patients are major stakeholders in implementing a project to reduce call light response times. When choosing stakeholders, also consider who you need to reach out to in the hospital and communicate information about the project. Regarding the call light project, HR staff are stakeholders because they will need to know about the project given their role of leading the hiring of new CNA staff. Describe at least four stakeholders in one paragraph. Use at least one source to support why these are key stakeholders. C. Barriers : What might prevent the change from being implemented? Who might not agree with the change or be dissatisfied with it (e.g., think about how the RN staff might respond to call light protocol changes)? How will you communicate with those who might not agree with the proposed change or have concerns about some aspects of the change?
