HSE 210 Module One Worksheet Assignment Guidelines and Rubric For this assignment, you will use the case study about Jean, provided in the Final Project Guidelines and Rubric document. Like many people treated in the healthcare system, Jean has various conditions that need attention. Here is an opportunity to begin thinking about Jean’s experience and the professionals and settings that have been or will be part of her care. Using the case study, outline the different healthcare settings and people that have been and will be a part of Jean’s care, including the human services professional . Describe the types of services they may have provided to her. To complete this assignment, use the worksheet below. First, identify four conditions or issues that are part of Jean’s experience and may require attention in the healthcare system. For each condition, identify tasks to be completed by the human services professional and by one other professional. Then identify two settings where each condition may be engaged and the type of work likely to be completed in that setting. Jean’s Conditions Condition 1: Severe stroke causing paralysis Condition 2: Assistant living due to needing round the clock c are with basic living care Condition 3: PTSD Post Traumatic Stress Disorder Condition 4: Li ving by herself Human Services Professional Tasks Helping with medical needs a long with feeding and having access to proper health care. Jean needs help with getting around her living space along with eating, bathing and getting dressed. Finding counselors Jean could visit and speak too, proper living conditions, helping provide safety from intruders. Help secure safety measure for house, helping with transportation needs, finding in home care. Manage daily/weekly a ppointments. Other Role and Tasks Having a plan put in place for when she is released along with follow up appointment. Physical therapy for the paralysis. Counseling for her stroke trauma and help with her daily tasks. Weekly counseling visits, support groups so she doesn’t feel alone, housing in a safer and more secure area. Obtain and install security system and camera s. Setting 1 and Work Performed Hosp ital can help with managing her medicine routine and other medical needs that occur. Hospital can help Jean with her physical therapy along with reaching the health care goals needed. Hospital can have a counselor visit there as it is a more secure place than visiting Jean elsewhere Nursing home will allow her to feel safe until she is able to do things on her own. Setting 2 and Work Performed Nursing hom e has the ability to help her with appointments and medicine. Nursing home capable of helping with anything simple that may still cause Jean problems. Rehab facilit y can help with Jeans PTSD and even help her learn proper coping. Finding a secure place to live and only letting family know where she lives currently. Ensuring her safety from intruders.
