What is the purpose of a treatment plan? The purpose of the treatment plan is to have a blueprint for treatment going forward. The treatment plan will act as a path that will be followed by the client and the treatment team. Treatment plans can be revised at any time either at the request of the client or if a treatment provider feels that the current plan needs revision due to either the progression or regression of a client. However, a client has the final say in the treatment plan that will be followed during their stay at a facility or outpatient treatment. In Johnathan’s case, it would be beneficial to address the mental health concerns as well as the drug and alcohol addictions. We will also need to help him address the backpain with a doctor so he can go back to work. Who is involved in the treatment planning process? The people involved in the treatment planning process are at the client’s discretion. It is their plan, and they must agree to all aspects of the plan. A treatment provider can make suggestions about who should be included in a treatment planning meeting. Ultimately it is the client’s decision on who will be included in the treatment planning process. In the case study of Johnathan, I would suggest that his mom be involved in the treatment planning if he was ok with her being present. He does not appear to have a close relationship with his siblings and since they do not live close, it does not make sense to include them unless Johnathan feels it would be beneficial. How is the presenting problem addressed in the treatment plan? The treatment plan for Johnathan should include a detox program first to address all his withdrawal concerns with the possible use of medically assisted treatment. He will need to address his back pain while in detox as well. Once this is completed an inpatient stay should be completed to look at what life will be like after being sober. There should be a conversation about continuing with the medically assisted treatment. Addressing his mental health while inpatient will also need to be considered. How goals are developed and the requirements of these goals. Each goal must be developed with the client’s mental health and substance abuse conditions in mind. The major goals in treatment planning are esthetics, function, and stability. In Johnathan’s case, I would discuss goals like dealing with his back pain, building better communication skills with family, and attempting to make sober connections in the community. Connecting with a peer specialist, and a vocational specialist before heading back into the community. How do you decide which problem to address first?
The treatment plan is designed and decided on by the client. Once the goals are determined I would work with the client to prioritize them. With Johnathan, I would address the back pain first with a pain management provider during detox. Second, I would address substance abuse issues and his depression.
