N132 CULTURE SEMINAR POWERPOINT GUIDED QUESTIONS Slide 2 1) What are your concerns for Ana and her family? One of my concern for Ana is her answering “yes” to all the questions. As she speaks no English, and only a little Spanish, it is hard to determine if our client fully understood what was being said about her condition and her baby’s. 2) What ethical issues do we see here? The ethical issues in Ana’s case include the language barrier in obtaining the consent and understanding of the materials and the validity of all the “yes” Ana answered to. Slide 3 1) What are some possible cultural factors that could have contributed to Ana not receiving the appropriate standard of care (Per American Medical Standards)? Cultural factors contributing to Ana not receiving the standard care, according to the American Medical Standard include her language barrier, being Hispanic, their job and background, etc. 2) What other factors were present that contributed to Ana not receiving the appropriate standard of care? Some other factors also include their lack of knowledge, having no health insurance, possibility of not knowing who to ask for help, etc. Slide 4 1) Are there any cultural conflicts and if so, what are they? In this episode, Ana may show very little emotion, but we are not aware of her real thought processes. And the husband Hugo is also seen asking for a traditional healer for the baby’s health and healing by being cleaned with an egg. 2) How could have this been handled differently? During this situation, considering the distress the father and the relatives were in about the baby, the nurse could have used that request to ask more about their belief and request instead of seeming visibly uncomfortable and dismissing it as an “alternative” treatment. 3) While Ana is in your care, what are some culturally relevant interventions you can incorporate to promote safe and quality care for Ana and her baby? Based on Ana and their family’s cultural beliefs, I could ask and listen to more about what Hugo had to say, and if possible, help achieve that request for them as it is a sensitive and stressful time for both the baby and the parents, and also adding the relatives. I could also incorporate any additional questions like, “Do you have any other cultural beliefs that we could help you achieve that helps the mother and the baby?”, “Are there any healers in your community who might be able to help you?”, etc. Slide 5 1) How could cultural barriers in this case cause Emma’s care to be compromised? As a Black American in between the doctors of European descents, it is natural to think one side knows nothing of the other’s cultural and/or psychosocial beliefs. Presumably as a Black American widow, a mother of four, and having to travel miles away for treatment, Emma is scared with no one of the same cultural background near her to help her feel safe. 2) What other concerns do you have for Emma?
With her fear that she may be used for experimental purposes without her consent, she may choose to leave and not to have the treatment, further instigating her disease to progress. And on the contrary that the facility was only option near me even within 225 miles away, finding another place that would also satisfy her financial and familial situation could be hard. 3) How could those concerns be addressed? We could help Emma to contact a social worker to help with her needs of financial situation, having to travel far for treatment, and her fears as a Black American away from her rural community. Slide 6 1) What cultural beliefs and values could be contributing to Lolli’s unwillingness to admit her sexual behavior in front of her mother? As a Southern Indian myself, I am able to understand Lolli’s emotions on answering those questions. As Indians, almost all the families hold a cultural belief of “Never sexually active” till after marriage. And parents, mostly both parties are patriarchal. 2) How could the health care provider request that Lolli’s mother leaves the exam room without offending her or implying that Lolli is hiding something from her? The health care provider could ask the mother to leave the room so that they could a physical exam on Lolli and that the mother may go with the nurse on any information she wants to know. 3) How could the provider successfully address Lolli’s health concerns and provide Lolli with sexual health information without causing a great amount of family discord between Lolli and her mother? The health care provider, after asking the mother to leave in a discreet way, could help Lolli to talk, allowing her to answer questions honestly, and educate her on her concerns on her sexual life. The provider could then educate her on the things Lolli wants to know about, while also keeping an eye on Lolli’s familial situation. The provider, consider the client Lolli’s as the main goal, could either be discreet in a way that could help Lolli greatly. Even though she is a minor, as the provider’s client, they are responsible to holding Lolli’s concerns as a secret also while thinking how the mother may react. Slide 7 1) How does understanding and acknowledging different cultural views help contribute to the goals of the Patient’s Bill of Rights and Responsibilities? Understanding and acknowledging different cultural views, to a health care provider, helps them to understand the client better and also understand why clients’ decisions are what they are without questioning or being uncomfortable about it. As providers, we give patients our priority, therefore, building a relationship with trust and rapport is essential in helping the patient receive the best quality of care.
