Burns, BSW Part I Critical Assessment of Client Reason for services starting now (Why Now): Mr. Juan Alverez, a 45-year-old Mexican American married male who has three children; recently lost his job. Mr. Alverez has been lacking interest in daily life since his job was lost. As a Mexican man Mr. Alverez reports to feelings of inadequacy when it comes to providing for his family and being the man of the house. Mr. Alverez has been following job leads but has not had success in obtaining new employment. Mr. Alverez has been self-isolating from spending time with his family, he has been spending time in his room. Mr. Alveraz has a support wife who encouraged him to seek help because she recognized his lack of interest in life and described his current behavior as moping around the house . Mr. Alverez s primary care physician referred him for service because he is suffering from depression. Exploring internal factors of the client: List the internal strengths of the client: Positive thoughts of self - “I am a hard worker,” “I am competent,” “Family is the most important thing in life” Strong cultural beliefs Good physical health Clearly defined goals (find employment) List the internal challenges of the client: Negative thoughts of self - “A good man is a provider —I am not a provider,” “I have let my family down —I am useless”. Loss of interest in social/family life Unrealistic or high expectations of self Doubt regarding obtaining employment Of the strengths and challenges listed above which of those strengths and challenges will you focus on in treatment and why? Mr. Alverez has very strong cultural beliefs. In the strengths and challenges list above there are both negative and positive aspects of his cultural beliefs. This is a very important part of who Mr. Alverez is and how he identifies as not only and individual but also a man, husband, father. This is something that I will focus on in treatment with Mr. Alverez. Mr. Alverez feels like less of a “man” because he does not have a means of income to support his family. Mr. Alverez needs support in achieving his goal of finding new employment. This is a goal that I will focus on during treatment with him. Exploring external factors of the client: List the external strengths available to the client: Family support Decent severance package College educated Dual language speaker 23 years of work experience List the external challenges affecting the client: Lack of employment Has stopped searching for employment Of the strengths and challenges listed above which of those strengths and challenges will you focus on in treatment and why? Mr. Alverez has a supportive wife who cares for his needs and well-being. This is a significant strength in his life that is important to focus on during the treatment process. His wife can support him through this process and maybe even be a part of some sessions in order to better understand her husband’s needs. Finding employment can be a fairly difficult and daunting process for any individual. In Mr. Alverez ’s case he is college educated with a master’s degree in engineering including 23 years of on-the-job experience. This is a great strength for Mr. Alverez as it makes his job prospects more attainable and accessible to him. Mr. Alverez believes that a mans place is to provide for his family as he man of the house. Without employment he feels like a hinderance to his family. Due to his lack of employment he is struggling with self-worth and with every missed job opportunity he has lost hope in the system of it all and has stopped trying. This is a challenge that needs to be focused on in session with him and the overall treatment process. Please explore (not just list) the cultural, spiritual, gender, sexual, racial factors that you identify as important to consider in this case?
2 Mr. Alverez has strong connections to his Mexican heritage. It has guided his beliefs about who he is as a man and how he views himself. Mr. Alverez expressed that the man is supposed to be the provider for his family. Without a job and no way to provide he does not feel like a man as he describes. Mr. Alverez has maintained his ties to his culture in the food, music, and family dynamic. Family is extremely important to Mr. Alverez and his Mexican culture and upbring play a big part of that. What do you hope will be different in the client’s life after they conclude treatment? I hope that Mr. Alverez will recognize his self worth as a husband, father, and man. I hope that Mr. Alverez will take pride in who is and the life he has been able to provide to his family thus far. What are the areas you plan on addressing in treatment (thoughts, behaviors, introduction of skills)? During Mr. Alverez ’s treatment I would like to focus on addressing the negative thoughts that he has regarding not being enough for his family. I would also work with Mr. Alverez to improve his interviewing skills and job application process. Lastly, I would like to work with Mr. Alverez to re-engage with his family and conclude with self-isolating behaviors. All of which will take time and everything with come as small steppingstones. Based on the above information and anything else you consider important to the case what potential areas client may want to address in treatment: Mr. Alverez has very strong cultural beliefs, and it would be beneficial to further explore his beliefs in this area. It would be useful for me as a clinician to conduct further research regarding the culture outside of the treatment. Additionally, inquiring more about Mr. Alverez s past and upbringing. Mr. Alverez needs healthy coping skills to better address his feels of self-doubt and negative thoughts. Developing healthy coping skills with him is very important. Mr. Alverez has several strengths that he seems to muddle with negativity. I should like Mr. Alverez to work on developing a list of the strengths that he believes he has. This would be an effort to assist him with developing a better resume and build his confidence when he goes on job interviews. Part II- Critical Assessment of self (your preparedness to work with this client) What are your own strengths and competencies that will be useful in assisting the client and providing treatment in this case: I have experience with creating behavioral plans to address a target behavior and replace it with an alternative behavior. This is something that will be useful when addressing Mr. Alverez current behaviors about negativity about himself. I have worked with vulnerable adults who are disabled members receiving division of developmental disability services for that last 12 years and continue to do so. I assist them with developing realistic goals for their life and work on objectives and steps in achieving them. What areas of this case would you want to address in supervision and receive guidance: I have limited knowledge regarding the Mexican culture and should like to better understand Mr. Alverez s beliefs about himself as a man. How will you address and manage similarities and differences between you and the client? I do not believe that Mr. Alverez and I will have many similarities. The only similarity I would see being a concern is that Mr. Alverez identifies as a Mexican American (first-generation). I identify as a Romanian American (first- generation) I have very strong cultural beliefs as well. However, on every other level Mr. Alverez and I could not be more different. “Curiosity Flags” – What are items for you to be aware of and consider as you move forward with this client. Does his extended family know how Mr. Alverez is currently doing? How has his self-isolation impacted his relationship with his children and wife? What positions of employment is Mr. Alverez seeking to obtain? Is he willing to take a lesser paying job and working his way up?
