The healthcare industry employs a population health approach in various ways to implement its
strategies. Preventive measures play a crucial role, encompassing vaccination campaigns, health
education, and screenings for the early detection of diseases. Collaborating with communities is
another key aspect, with programs that promote healthy living and safe practices often being
community-based, recognizing the influence of social determinants of health. Data analytics is
integral to population health management, allowing healthcare organizations to identify health
trends, risk factors, and areas for improvement. This data-driven approach helps tailor
interventions to specific populations. Additionally, the shift towards value-based care models
reflects a commitment to keeping populations healthy rather than solely treating illnesses,
aligning with the NQS goal of making quality care more affordable for individuals, families,
