SITXWHS003 A3 TASK 1 RISK ASSESSMENT TEMPLATE Situation 1 : ABC company has recruited new employees in the last 2 years in different positions at its 5 levels of work building. In the last 2 years, ABC company has not done any fire drill as required by law for the safety of all staff at workplace. The building had been renovated to include many offices as well as more exits; however, the evacuation map has not been updated to include the new development in the office building. Some of the fire alarms were checked but not all, however, nothing has been documented in any record for future reference. Potential hazards Potential risks Who is at risk? Existing control measures Preventative measures Responsibilities Situation 2 : ABC company maintains a safe at the workplace for petty cash only. It is required for Mr. Stone - account officer to make bank deposit everyday at 3pm. Due to busy tasks every afternoon at his office, Mr Stone shoves cash in his draw, and sometimes forgets to lock the safe, as he dashes to the bank to do his banking task before the bank closes for the day. He is supposed to do banking report and at the bank, however, he, sometimes, leaves this task to the following morning. He realises that cash go missing from his draw and safe as he does his report and reconciliation report. Unfortunately, he has to pay the difference, which usually costs him more than half of what he’s earning in a week. He starts to blame his co-workers, even those that wouldn’t even come to his office for any reason. Potential hazards Potential Risks Who is at risk? Situation 3 : Mr. Smith, the manager for ABC company, understands his WHS role as an employer – to make sure the work environment and equipment are safe for all employees. He has incident report form in place to any incident of injuries or WHS related issues. He put up all relevant signs as required by law. However, Mr Smith has not done any WHS meetings, or set up WHS committee or WHS officer in the workplace to be in- charge of WHS issues but only himself. Some loosing electrical wires were lying around the walkways, the rain has caused some areas in the office to be slippery yet not sign was put on this. As a result, one staff member was walking through the wet area and slipped. He twisted his right hand and was not able to work for 2 weeks. Unfortunately, Mr Smith was away on that day. The supervisor was trying so hard to understand and to fill the incident report, and at the same time, trying to assist the injured staff. The staff was contemplating in suing the company for lack of practicing WHS responsibilities. Potential hazards Potential risks Who is at risk?
