Nick has been drinking since the age of 10, Nick is now 17 years old he has been drinking for about 7 years but it known that he started drinking more excessively at the age 14 so that’s 4 years of excessive drinking. Now Nick is 17 and he is actively seeking help.
Dimension Two: Biomedical conditions and complications Mild -Nick did not report any biomedical conditions however that doesn’t mean that there are no issues so mild was marked Dimension Three: Emotional, behavioral, and cognitive conditions and complications Moderate-Nick has not much support or guidance throughout his life. His father he never met due to him leaving before nick was born. Nicks mother works three jobs to financially support them but that could mean that she hasn’t been able to meet Nicks other needs. At the age of 10 Nick joined gang because he needed to be safe. He also started drinking at the age10 and that increased as he got older at the age of 17, he was expelled from school after being caught with alcohol on him in school. He dabbled with drugs before he realized that he needed and asked for help. Nick has emotional in that he supports and safety he aware of is from his friends in the gang. He shows impulsive behavior in bringing alcohol to school even after he was previously and Nick only 17 his brain and cognitive functions are not fully developed. Dimension Four: Readiness to change Moderate-Nick at his current age of 17 has reached out for help and states the knows he needs help but doesn’t have support or guidance. His willingness to change is dependent on his environment the people in his environment. the gang culture can have an impact on his readiness to change. Dimension Five: Relapse, continued use, or continued problem potential Substantial- Nick is only 17 years old he is doesn’t have support other \than his mother who works three jobs and admits she unsure how to help him, Nick lacks a positive role modals and is an environment with I the gang that is unsupportive and could potentially lead to a relapse ,continued use or a continued problem. Dimension Six: Recovery/living environment Severe- Nicks recovery is dependent on his environmental support. Nick needs an environment where he feels safe and supported. In his current leaving sudation this might mean giving additional support to Nicks mom so she can learn how she can be apart of his
recovery if she chooses to. This might also mean working on getting Nick out of the gang safely and finding additional supportive such as therapeutic groups and educational or vocational training .These can give him the support and a sense of purpose .that is currently lacking . Reflection: This assessment has made it clear that in working with nick as impotent as it will be to treat the drug and alcohol se disorders a critical part of Nicks treatment plan will be finding resources to create a safe and supportive environment for nick .
