2 Single-Subject Design Study Introduction Paula is a 43-year-old multiracial female who was diagnosed with HIV, Hepatitis C, and is diagnosed with bipolar disorder. Paula faced many adversities during her life, including abuse, illness, and injuries. Paula currently lives alone in subsidized housing and no family has been reported. Paula does attend the local church in the area where she lives, but with her sickness, her attendance has not been very far off. Paula found out she was pregnant from an acquaintance. She’s not sure if she wants to keep the baby or terminate the pregnancy. The father is abusive, and Paula is in fear of her life for him. The father of her unborn child stalks her and makes unwanted visits several hours of the day. All of this happening to Paula, made her start back smoking to calm her nerves. She also stated she stopped taking the medication for HIV. Literature Review Paula was diagnosed with bipolar disorder, Hepatitis C, and was previously diagnosed with HIV, which caused her life (Plummer Markis & 2014). The prescription that was prescribed to Paula due to her bipolar disorder has caused her to be depressed and have episodes of mania. However, she has been consistent with her medication. Paula did decide to seek help with the pregnancy, understanding her clinical conditions and living situation could affect her unborn child. Paula’s social workers help her to pinpoint the father of her unborn child. Understanding Paula’s fear of the child's father's abusive nature along with the use of drugs has Paula in fear. Paula's Interdisciplinary team is working together on a plan that will address all of Paula’s issues. The psychiatrist stated that Paula had been noncompliant with her medications, stopping them from realizing the way they made her feel. Right now, Paula’s current issues such as medication management, and her ability to care for unborn children are inclusive.
3 Methodology The first concern that I would address would be to ensure that Paula has a secure and safe place for her and her unborn child as well as obtaining a restraining order. Paula only had one incident with him stalking her, but I would rather be safe than sorry for her safety and her unborn child. I would also find a domestic abuse shelter to help Paula connect with others who had similar issues. One major issue I would address is having the fear of taking medication properly. I would get the proper education assistance on the importance of taking her medicine correctly and how if she doesn’t take her medicine correctly if could affect her unborn child and could more than likely have a high percentage to contract her unborn child with the HIV virus. Paula has an understanding that she needs to stay healthy for her and her unborn child. She understands the bigger picture and that takes care of herself mentally and physically. Setting and Intervention The model I felt that I would use to track her progress during the intervention process is the "Goal Attainment Scale (GAS) 5-point system" (Dudley, 2016) effectively. The GAS was originally designed to assess the outcome in a mental health setting, however now it offers several potential advantages. The GAS is built upon goals that have already been set at the beginning of the intervention. However, when tracking a client's progress, it should be tracked in the beginning and in the middle to see if any adjustment to the intervention is needed, a process has been made. Another tool I would consider using with Paula is the Psychiatric Rehabilitation Process Model. A client-centered strength-based model (SAMHSA, n.d.). This tool will help educate Paula with other available resources to help improve her independence, self-determined goals, and self-care. Through the Recovery Support Strategic Initiative and another effort, SAMHSA supports the high quality, self-directed, and satisfying life in the community for all

4 people in recovery, and includes health, home, purpose, and community (SAMHSA, n.d.) This could improve Paula's ongoing training in self-care, medical maintenance, her connection to social services, and her trust in receiving services from the system. This intervention points to the importance of helping Paula maintain her housing, mental health, stability, continued treatment, and successful recovery (SAMHAS, n.d.). Her progress depends on herself. Participation ranges from 6 months to 2 years depending on independence and safety development. Single-system research (SSRD) includes studying a single individual or a single system, gathering repeated measurements, and identifying variables (Dudley, 2016). Single- System Research (SSRD) is used to monitor and evaluate participants' responses to the interventions with which they address participants' target problems (Dudley, 2016). SSRD charted measurements of individual behaviors while graphing the client's progress, change, and improvements (Dudley, 2016). Expected Results Shortly, in Paula's case I would expect her to participate in a service that would incorporate strategies that will target healthier habits and medication compliance. The Psychiatric Rehabilitation Process Model would provide strategies for Paula's treatment plan. The individual session would remain in place and depending on the severity of Paula's current issues will determine the frequency of session being considered.
