ABCD’s of Cultural Assessments Attitudes of Client and Client’s Social Support Network What attitudes does this ethnic /cultural group in general – and the client and other social supports (family/friends in particular) – have about truth telling with regard to diagnosis and prognosis? Do they have positive or negative attitudes about aspects of mental health care? Are the client’s social supports supportive of the assessment process? “What does this having this assessment done mean to you? What meaning would it provide for your and your family? Do you have any thoughts or feelings about receiving a particular diagnosis?” “Have you ever been in therapy or met with a psychologist before? If so, what was your experience like? What was helpful or unhelpful? Do you have any questions about the assessment process?” Beliefs What are the client’s beliefs about mental health or diagnoses? What beliefs/values are moving them toward obtaining/completing an assessment? “How can we support your needs during this process? Who would you like to involve in this process?” Context Determine the historical and political context of the patient’s and family’s lives, including place of birth, refugee or immigrant status, poverty, experience with discrimination, health disparities, language spoken, and degree of integration within their ethnic community and the degree of assimilation into Western culture. “Where were you born and raised? How long have you lived in the United States? What has your experience been since coming to the U.S. (or the city)? How has your life changed since coming to the U.S.? What language are you most comfortable using when talking about your mental health care? What were other important times in your life that might help us better understand your situation or background?” Decision Making Style What is the general decision-making style of the cultural group and specifically of the client and family (or anyone else involved in the client’s mental health care). Is the emphasis on the individual decision making process or the family decision making process?
ABCD’s of Cultural Assessments “How are decisions about health care made in your family? Is there anyone else I should talk to in your family about your diagnosis or assessment results? Who would you like to attend the feedback meeting/obtain a copy of the report?” Environment What resources and support are available to the patient and family? What resources are available to assist the health care provider to interpret the significance of the patient’s culture? Identify community resources that may be of assistance to the provider, client, and family such as translators, health care workers from the same community as the patient, community associations, religious leaders, and other healers.
