PSY 315 Milestone One Case Study: Alicia is a 22 year old web designer. In early November she started to feel blue and she was finding it difficult to be motivated to do much. For the first couple of weeks, Alicia rationalized that her gloomy feeling was due to the increased darkness from daylight saving time which always made her feel a little sad every year, even when she was a little girl. This time, Alicia was not coming out of it like she had in the past. Her lack of motivation was increasing and she was having a hard time “faking it” to friends and family. Although Alicia is usually a fairly happy and optimistic person, since November she has convinced herself that other people have a more meaningful life than she does. Activities that Alicia used to enjoy became uninteresting and a chore, so she began doing them less and less. She no longer accepted invitations from her friends to go out shopping and dancing, two of her all time favorite things. By the middle of February, Alicia would only leave the house to go to work although she found it a huge effort to do so and thought everything she worked on was “meaningless”. She began to feel hopeless about the future, and was worried about losing her job because of her poor attitude and high absentee rate so she decided to take two weeks off to try to pull herself together. Alicia went to her family doctor and was prescribed anti-depressant medication. A month later, nothing had changed. These are the reasons why she is presenting for therapy today.
PSY 315 Milestone One Patient Background: Alicia is a 29 year old email engineer presenting feeling of depression and is isolating from others. She has a consistent feeling of hopelessness and is struggling to find meaning in her life. She is experiencing a lack of motivation to engage in daily life activities. Activities that user to find her joy are no longer joyful for her. Alicia states that she believes these feeling are due to the daylight saving time. She states that she has had symptoms since a child during this time but, the would resolve on their own. This instance, they are not resolving. She is confused and frustrated that she can not shake the depression and continue with her daily life. Alicia is concerned that if she does not shake the feelings soon that there will be future consequences in her future with employment. “She began to feel hopeless about the future, and was worried about losing her job because of her poor attitude and high absentee rate so she decided to take two weeks off to try to pull herself together.”(Southern New Hampshire University, n.d). Her work is starting to feel meaningless to her. Her personal life is also suffering due to the depression, she is now declining invitations to participate in activities with her friends. She is hopefully that the two week absence from work with give her enough time to pull out of her depression. Before seeking the help of a therapist, she saw her family doctor. The doctor prescribed a anti-depressant. After a month on the treatment there has been no change to her condition. Alicia is presenting symptoms of major depression. “Major depression is a medical illness that affects a person’s mind, mood, body, and behavior. It is more than "feeling down" because of a recent loss or family, work or financial stresses. It occurs when these feelings become more intense and persist to the point that they affect daily functioning.” (NAMI, 2008). Treatment Plan:

PSY 315 Milestone One The theoretical orientation that I believe will best work with Alicia is Cognitive Behavior Therapy (CBT). “Cognitive therapy is highly collaborative and involves designing specific learning experiences to help clients understand the links between their thoughts, behaviors, emotions, physical responses, and situations (Greenberger & Padesky, 2016). The goal of CT is to help clients learn practical skills that they can use to make changes in their thoughts, behaviors, and emotions and how to sustain these changes over time.”(Corey, 2017). CBT will help Alicia become aware of her negative thinking, she will be able to view challenging situations in more clearly and respond to them in an effective manner. Cognitive behavior therapy will help her in dealing with her depression and focus on the challenges that trigger it. She will be able to improve her emotional regulation and learn how to better handle stress in her life, as well as her seasonal depression episodes. “From the start of treatment, clients learn to employ specific problem-solving and coping skills. Through a process of guided discovery, clients acquire insight about the connection between their thinking and the ways they act and feel.” (Corey, 2017).
