IEP Quick Reference For: Darian Mwambe IEP Summary Sheet for the Inclusive Classroom Student: Darian Mwambe Age: 14 Case Manager: Dannie Spann, Jessica Kenagy, Amber Kearney Introducing: Darian Mwambe is a 14-year-old student that immigrated to the United States from Rwanda. He lives with his extended family unit that includes his mom, aunt, siblings and cousins. His father remains in a refugee camp but talks to Darian weekly. Darian’s dad taught him how to read, write, about history and math, when Darian was not able to attend school. He enjoys school, church and has lots of friends. Eventual Darian and his family would like to gain citizenship and have his father rejoin the family in the U.S. He also enjoys reading comic books, watching movies, playing basketball and cheering for his favorite basketball team, Chicago Bulls. LRE: Darian should be in general educations classrooms with an associate aide for his mild hearing loss and to discriminate similar sounds, particularly when hearing new vocabulary. Some of his struggles come from his physical disability of having a cleft palate, lip and a mild hearing loss. Where we’re at: Academic Summary: Strengths : His reading skills are at or above grade level and he quickly masters oral motor adjustments for tapping sounds (t/d) and lifters (r,l). Needs : Producing sounds involving the cleft/lips (p, b) nasal sounds (m,n) and sounds made through contact with the rear palate (g,k). Strategies the student may use: Practicing, build self-confidence, feedback, supportive parents, advocate for needs/concerns/progress Strategies the teacher may use: E nsure there is good oral pressure during sound production and establish new motor speech patterns that replace speech sound errors. 1
IEP Quick Reference For: Darian Mwambe Social/Behavioral Summary: Darian has had surgery to correct his cleft palate and working to adjust to swallowing skills along with academically affected skills. He shows progress with his ability to swallow different items based on thickness. Darian is grateful for the surgery and the therapy but lacks confidence in the overall results. He becomes very frustrated because others still struggle to understand him and he grows impatient. He prefers not to read aloud or participate in large group because of his inaudible communication during discussions. Darian will discuss topics in class with the teacher or a small group of preferred peers/friends. His private therapist is aware of his social concerns and monitoring his self-esteem issues. Where we’re going: Goal and objective 1: In 36 weeks Darian will be able to produce sounds involving the lips (p, b), nasal sounds (m, n), and sounds made through contact with the rear palate (g, k) with 80% accuracy in 3 out of 4 trials. Darian will continue working on these skills with his speech therapist 30 minutes per week. Darian will increase time with his speech therapist if needed. When this goal has been met Darian will then be able to produce sounds involving the lips (p, b), nasal sounds (m, n), and sounds made through contact with the rear palate (g, k) with 90% accuracy in 3 out of 4 trials. Your focus: Speech Documentation responsibilities: Speech therapist, general education classroom teacher or the special education teacher will collect data. Goal and objective 2: In 36 weeks Darian will gain the confidence to participate in group discussions or reading in front of the class. He will volunteer to speak or read 5 times per week in 3 out of 4 trials. When this goal is met, Darian will then volunteer to speak or read 8 times per week in 3 out of 4 trials. Your focus: Social Skills Documentation responsibilities: General education classroom teacher, special education teacher or the paraprofessional will collect data. Goal and objective 3: In 36 weeks Darian will be able to try new foods with different textures and be able to chew and swallow them with 85% success in 3 out of 4 trials. When this goal is met, Darian will then be able to try new foods with different textures and be able to chew and swallow them with 95% success in 3 out of 4 trials. 2
