Example: Client is a 13-year-old single Caucasian female.) Jared is a 5-year-old Haitian American child, who was referred by his school's principal, his teacher, and his mother for concerns about aggressive behavior, destruction of property, foul language use and poor social skills at school and at home. Jared has no reported prior mental health history. He reportedly lives with both parents, his maternal grandmother, and his aunt. His mother reportedly states that he is having difficulty adjusting socially, especially with his peers. He becomes increasingly angry, appears frustrated and irritable, and most recently he has started to refuse to go to school. Presenting Concerns and Specific Problems His school's principal, teachers, and his mother for concerns about aggressive behavior, destruction of property, foul language use and poor social skills at school and at home. Concurrent Problems Jared's father being incarcerated changed the dynamic of Jared household causing worry and distress. Jared has been attending soccer practice, to help him cope with the loss of his father. Background History and Stressors (Current, Developmental, and Ecological) Jared grandparents have been married since 1980, they migrated to America with Jared's father John. John was born in Haiti in 1984. Jared grandfather worked a cab driver and maintenance man. Jared's grandmother Jocelyn was a homemaker, she made pastries and often sold them in the neighborhood for money. John was an only child for a long time, then his sister Johanna was born in 1995. They have a close relationship. Jefferson worked very hard to maintain his family and he passed away at the age of 40. Leaving John vulnerable and misguided. John turned to street life early onset. John met his wife at school. Janet is also of Haitian decent. but her parents died when she was a child. They married in 2003, and then started their family with the birth of Jared and Janice. Jared worked construction due to having limited education and sold drugs on side to maintain his family. This is the reason he is incarcerated now. Jocelyn has been very supportive of Janet helping with children due to changes in the household. Janet was a homemaker, and she 1

The Jean family includes: the loving relationships the immediate family share with one another, the church community with which they are involved, Jared’s school and there. Tenuous factors affecting the family include Janet's employment, Janet health and treatment, Jocelyn involvement, and Jared’ s soccer practice. Janet’s employment is both a positive and negative factor, as it provides much- needed financial resources, but it also keeps her away from her children almost all the time. Janet's currently is taking Valium to control her anxiety, which contributes to her functionality within society and her household. Jared's mother also attends a support group for other woman that have loved ones that are incarcerated. Being that the family used to attend church together, they would benefit the continual attendance at church services to promote solidarity amongst the household. Strengths (Protective Factors and Signs of Resilience) His Strengths/supports include supportive family and their awareness of his emotional behavioral needs, his family willingness to engage in individual and or family counseling. Interfering Factors or Barriers Jared has experienced many changes over the past several months. His father is incarcerated for 4 years and then he is facing deportation to his native country of Haiti. His family may possibly have to move due to financial reasons. Jocelyn Jared grandmother will move in with Janet and the children to assist her raising the children. Plans for Treatment (Goals) Treatment Long-term Goals: 1. 1.Jared will learn to identify any stressors that contribute to his negative emotional reactivity. 2.Jared will develop anger management, coping, and problem-solving skills to decrease physical aggression. Treatment Short-term Goals: 1. 1.Through psycho education Jared will learn to: communicate when he something is bother him, He will learn to manage his reactions in a positive manner. and/or conflict resolution skills. 2. Jared will learn a minimum of 3 self-regulation skills in 2
Week 7 Assignment Template group CBT to help him learn how to process and manage his thoughts, feelings, and impulsive behaviors. He will demonstrate these self-regulation skills for a period of 4 consecutive weeks. 3.Jared will engage in group play to learn to follow directions from adults without engaging in yelling or aggression for a period of 4 consecutive weeks. 4.Jared will learn and utilize at least 3 positive self-regulation skills in play therapy to express feelings without engaging in physical aggression in the classroom as demonstrated for a period of 4 consecutive weeks. Treatments should include individual counseling with an evidence-based approach such as CBT and Family therapy. As his counselor I would also suggest building his social skills as well. Prognosis is favorable, with anticipated benefit within 12 sessions of CBT 3
