BIPOLAR BIPOLAR DISORDER DISORDER PAST MEDICAL HISTORY CHEIF COMPLAINT Patient presents with an increase of nervousness, tremors, rapid-pressured speech, and impulsiveness. History of Present illness SOCIAL HISTORY Mrs. H was a full time worker at a local coffee shop but was recenty fired due to tardiness and forgetfulness. Mrs. H graduated highschool but has reported difficulty furthering her education and making friends. FAMILY MEDICAL HISTORY Her mother and father are relatively healthy aside from her fathers hypertension and her mothers frequent “ crying episodes.” Depression, Elevated Mood, Impulsivity, Mania/Hypomania, Mood Cycles, Risky Behaviors Mrs. H is a 33yr old female with a history of bipolar disorder and frequent manic episodes. She reports that as of a month ago she lost her job and has been living from one friends house to another. She states that her world is falling apart and she does not know how to get help. Lynn ann Blix Chamberlain University CURRENT MEDICATIONS Valproic acid - 750mg BID Risperidone - 2mg daily Trazodone - 50mg PRN HS
Mrs. H will require extensive treatment for her whole life based on her clinical presentation and diagnosis. A person with a chronic condition such as depression must be treated adequately, as often as necessary to achieve stability. The Tennessee Department of Mental Health and Substance Abuse offers a wide variety of services for persons with mental health disabilities. There are group counseling centers, counselors, and other services offered at no cost. As an NP it is essential to know what community resources are available. That way they can be shared with patients. NP’s need to recognize an increase in manic episodes or declines in daily functioning. Np’s can refer patients for treatment as both in and out patients. Patient’s can also be referred for counseling and other psychiatric treatments. WAYS TO HELP COMMUNITY RESOURCES HOW DO THESE FACTORS IMPACT THE PATIENT IN THE SCENARIO YOU PROVIDED? INTERVENTION OF MEDICATIONS T his patients medication regimen needs intervention. She appears to need dosing adjusting and an evaluation of medication compliance. TENNESSEE REGULATIONS Any practitioner that holds DEA clearance may prescribe psychiatric medications as long as they follow the prescribing guidelines and maintain supervision by a physician (TN.gov, 2023). AFFORDIABLE CARE ACT According to Novak, the Affordable Care Act promotes services for persons with psychiatric conditions at the same level of cost as any other condition. (2018). Tennessee law states that providers who do not adhere to practice guidelines will be charged with a felony and or face prosecution. HELP FOR MRS. H In this clinical scenario Mrs. H would benefit from group therapy and one on one counseling. Having someone to talk with to process emotions, especially during periods of high stress have higher outcomes of controlled breakthrough behaviors ( Malhotra , 2017) LEGAL & ETHICAL CONSIDERATIONS 1 1 2 2 3 3 4 4 5 5 6 6 7 7 8 8 Tennessee law requires that providers must keep specific details of persons with mental illness and submit cases to NICS. This ensures that persons with mental illness do not ever own a legal firearm (TN.gov, 2023). Failure to comply will result in fines, prosecution, and loss of licensing.
