HEALTH OUTCOMES FOR VULNERABLE POPULATION 2 Health Outcomes for vulnerable populations Despite various efforts in the world to eliminate healthcare disparities, significant discrepancies comprising risk components, healthcare inaccessibility, mortality, and morbidity still exist in vulnerable communities. For instance, many poor people, at times, do not get proper treatments due to lack of finances. Poverty and poor health correlate among the vulnerable groups. Vulnerable groups include the homeless, economically disadvantaged, emigrants, racial subgroups, disabled, the elderly, among others. Moreover, when stating vulnerable groups, it can mean subgroups at risk of accessing health services. In the case study, the vulnerable group is Johnson the 72 years old with a dementia diagnosis and his wife. Mr. Johnson wife cares for him, but she is also old and tired to continue with nursing services. That is the primary reason why she prefers looking for a nursing home. Dementia is a term that depicts categories of symptoms that leads to memory loss or decline of thinking skills reducing the ability of an individual to perform routine activities[ CITATION Mie16 \l 1033 ]. Dementia symptoms vary; however, the typical signs involve an inability to focus and paying attention, visual perception, poor judgment and reasoning, and communication difficulties. The vivid risk factors associated with Mr. Johnson case are environmental and psychological. Environmental risk element in the case range from social factor. The social factor can influence Johnson either positively or negatively. Johnson health may either improve in the new nursing home due to proper services or deteriorate due to seclusion effects of her wife. Conversely, the psychological risk revolves around age. Research state that dementia illness increases with age (Baumgart et al., 2015). Therefore, the more MR. Johnson age, the more

Frequently, Johnson my often forget essential activities such as meal times and medication that affect his health. While various risks affect the health impact of Mr. Johnson and his wife, Alzheimer’s Foundation of America can help this vulnerable group improve their living standard. Alzheimer’s Foundation of America in my community offers various services to people living dementia, caregivers, and the public. Its mission is to provide services and support and education to families, caregivers and people affected by dementia as well as funding researches that develops a cure and better treatment[ CITATION Alz19 \l 1033 ]. The agency works with dementia practitioners to provide training to improve the quality of services given to all patients. Acceptability AFA provides resources to families in the time of need. It operates nationally with a free helpline to prove ready information to all people. It has licensed social workers that assist and support callers and connect them with various resources in the specific areas regardless of where they live. Past experiences show the agency has more than 2800 organization in fifty states of the United States[ CITATION Alz19 \l 1033 ]. The national network has different organizations serving families affected by dementia-related diseases showing a high level of acceptability. Accessibility The agency has a memory screening initiative that provides free and confidential memory screen across the country.
